A Review of Evidence Supporting the Use of Anti-Biotics in the Treatment of Acute Otitis Media in Children to Prevent Mastoiditis.

...the treatment of Acute Otitis Media in children to prevent Mastoiditis. 4. Reference List 5. Appendices: i. SECAMB Clinical Management Plan/Patient Group Directive ii. FIGURE 3 - Thompson et al (2009)   A review of evidence supporting the use of Antibiotics in the treatment of Acute Otitis Media in children to prevent Mastoiditis. Student no 18830 Rationale The Paramedic Practitioner (PP) role has recently adopted a number of Clinical Management plans and Patient Group Directives (PGD) increasing the scope of practice. One such PGD (Appendix i) allows for the utilisation of antibiotics, namely Amoxicillin and Clarithromycin, for the treatment of Otitis Media (OM). This essay will look at the evidence supporting the use of these antibiotics, their efficacy and if early use prevents OM from developing in to Mastoiditis. Its primary aim is to enhance a PP’s knowledge and support education working towards clinical autonomy. Description Otitis Media literally means inflammation of the middle ear (Merriam-Webster 2011) and is predominantly caused by an effusion with in the middle ear caused by Eustachian tube dysfunction (Dhillon and East, 1994, pp 7). This dysfunction is caused by a relative obstruction of the Eustachian tube that creates a negative air pressure within the middle ear. Over time interstitial fluid is drawn in from the surrounding tissue resulting in the effusion (Natal 2011). Eustachian tube dysfunction often follows an upper respiratory tract infection...

...Queen’s University Belfast Queen’s University Management School. MGT2005 OPERATIONS MANAGEMENT Semester 2, 2012 “You don’t have to change. . . Survival is not mandatory.” (Professor William E. Deming) Course Co-ordinator: Dr Yu Xiong Introduction to Operations Management Operations Management (OM) is a functional field of management encompassing the design, operation and improvement of the processes and systems employed in the creation and delivery of an organisation's products and services. Essentially, operations management is concerned with explaining how factories and services work. Managing operations well requires both strategic and tactical skills and is critical to every type of organization, for it is only through effective and efficient utilization of resources that an organization can be successful in the long run. A few common threads pervade the material we cover in this course:  First, decision making in operations is highly context dependent. Recognising this is half the battle in managing an operation.  The variety of operational contexts obscures the link between particular operational decisions and overall firm profitability. Making this link transparent is essential to making better operational decisions.  Variability exists in any operation, and this variability needs to be managed  Integration, both across functional areas within a firm, and across firms in the value creation and delivery network, is crucial for superior firm performance...

However, the implied volatility surface also changes dynamically over time in a way that is not taken into account by current modelling approaches, giving rise to ‘Vega’ risk in option portfolios. Using time series of option prices on the SP500 and FTSE indices, we study the deformation of this surface and show that it may be represented as a randomly fluctuating surface driven by a small number of orthogonal random factors. We identify and interpret the shape of each of these factors, study their dynamics and their correlation with the underlying index. Our approach is based on a Karhunen–Lo` ve e decomposition of the daily variations of implied volatilities obtained from market data. A simple factor model...

...The Ethical and Legal Complexity of Medical Tourism: Questions of International Justice, Economic Redistribution and Health Care Reform Professor Sarah McBride Toro Longe April 18, 2010 M.J. Thesis in Health Law Abstract This is an essay on medical tourism in the United States of America (U.S.). It includes a brief history of the U.S. health care system, examines the social, cultural, ethical, and legal issues that have affected health care changes in America. With the number of Americans going overseas to seek medical care steadily rising, the American insurance industry expanding benefits, and the Joint Commission accrediting facilities for globalization of the health care marketplace, medical tourism should become increasingly important in the health care industry. While there are many reasons for patients seeking health care outside of their own country, one that is particularly easy to overlook is the outsourcing of health care and its effect in U.S. and around the world. The purpose of this paper is to examine the concept of medical tourism, noting the specific medical tourism destinations, presenting reasons for the recent increase in medical tourism, and examining the risks and benefits, as well as wrestling with the challenging ethical and legal issues inherent in medical tourism. The paper will conclude with consideration of the role of the law in medical tourism. Introduction Over the past 100 years, the United States of America (U.S.) has changed greatly...

A leak of methyl isocyanate gas and other chemicals from the plant resulted in the exposure of hundreds of thousands of people. The toxic substance made its way in and around the shantytowns located near the plant.[2] Estimates vary on the death toll. The official immediate death toll was 2,259 and the government of Madhya Pradesh has confirmed a total of 3,787 deaths related to the gas release.[3] Others estimate 3,000 died within weeks and another 8,000 have since died from gas- related diseases.[4][5] A government affidavit in 2006 stated the leak caused 558,125 injuries including 38,478 temporary partial and approximately 3,900 severely and permanently disabling injuries.[6] UCIL was the Indian subsidiary of Union Carbide Corporation (UCC), with Indian Government controlled banks and the Indian public holding a 49.1 percent stake. In 1994, the Supreme Court of India allowed UCC to sell its 50.9 percent share. Union Carbide sold UCIL, the Bhopal plant operator, to Eveready Industries India Limited in 1994. The Bhopal plant was later sold to McLeod...

Brissimis Bank of Greece and University of Piraeus Matthaios D. Delis Athens University of Economics and Business ABSTRACT The aim of this study is to examine the effect of bank-specific, industry-specific and macroeconomic determinants of bank profitability, using an empirical framework that incorporates the traditional Structure-Conduct-Performance (SCP) hypothesis. To account for profit persistence, we apply a GMM technique to a panel of Greek banks that covers the period 1985-2001. The estimation results show that profitability persists to a moderate extent, indicating that departures from perfectly competitive market structures may not be that large. All bank-specific determinants, with the exception of size, affect bank profitability significantly in the anticipated way. However, no evidence is found...

..."fair use," that user may be liable for copyright infringement. How to Say Nothing in 500 Words Paul Roberts Paul Roberts (1917-1967) was a linguist, a teacher, and a writer at San Jose State College from 1946 to 1960 and at Cornell University from 1962 to 1964. His books on writing, including English Syntax (1954) and Patterns of English (1956), have helped generations of high school and college students become better writers. "How to Say Nothing in 500 Words" is taken from his best-known book, Understanding English (1958). Although written almost fifty years ago, the essay is still relevant for student writers today. Good writing, Roberts tells us, is not simply a matter of filling up a page; rather, the words have to hold the reader's interest, and they must say something. In this essay, Roberts uses lively prose and a step-by-step process to guide the student from the blank page to the finished essay. His bag of writing su;ptegies holds good advice for anyone who wants to write well. PREPARING TO READ How do you feel about writing? Do you find writing difficult? Religious Experience

...Essay RELIGIOUS EXPERIENCE IN THE AGE OF DIGITAL REPRODUCTION* Frederick Mark Gedicks† Roger Hendrix†† (forthcoming in St. John’s Law Review (Fall 2004)) And the angel of the Lord appeared to Moses in a flame of fire out of the midst of a bush; and he looked, and lo, the bush was burning, yet it was not consumed. God called to him out of the bush, "Moses, Moses!" And he said, "Here am I." Then he said, "Do not come near; put off your shoes from your feet, for the place on which you are standing is holy ground." And he said, "I am the God of your father, the God of Abraham, the God of Isaac, and the God of Jacob." And Moses hid his face, for he was afraid to look at God. —Exodus 3:2, 4-6 Now as Saul journeyed he approached Damascus, and suddenly a light from heaven flashed about him. And he fell to the ground and heard a voice saying to him, "Saul, Saul, why do you persecute me?" And he said, "Who are you, Lord?"
