Antionette is a 38yo, G4 P1111, who is currently 7 weeks 0 days by LMP. She has not had any ultrasounds this pregnancy. She had symptoms of a UTI and was diagnosed at her PCP with both UTI and a urine pregnancy test that was positive. She was referred to our office due to a poor history. She currently is on Macrobid for the UTI. Her obstetrical history is significant. In 2002, she had a pregnancy that she states she was in/out of the hospital for with int4ermittent bleeding. She never required a blood transfusion but does report some significant bleeding. Ultimately at 24 weeks she says she went into labor and they were unable to stop the labor and unfortunately the baby delivered around 24 weeks. It was stillborn at that time. She mentions both the word abruption, which I would assume would be a chronic abruption given the intermittent bleeding …show more content…
She also mentions the fact that she was told that her cervix may be insufficient to carry a pregnancy; however, she then had a full-term delivery in 2005 without any mention of the cervix making cervical insufficiency less likely. After that delivery in 2010 she had an elective termination and she is here today. She does have a notable history of fibromyalgia and chronic back pain. She reports that the back pain is arthritis in addition to her fibromyalgia. She has even had steroids injections in her back with some relief. She has been tried on gabapentin but that did not work and is currently being maintained on naproxen and Advil, both of which allow her to function and maintain a job. Upon finding out that her period was late she stopped these medications. She is trying Tylenol but it is having minimal relief.
