Once a client has completed the LARS general intake form the client completes the Self-Sufficiency eligibility paperwork with the SS Program Coordinator or with another case manager. If the eligibility paperwork is completed with the SS Program Coordinator then at this time the SS Program Coordinator will hold an information session with the client about the program entails and the requirements of participation. If the eligibility paperwork is filled out with another case manager the SS Program Coordinator will call the client and have the client come in for an information session after the SS Program Coordinator has reviewed the eligibility paperwork. Following the information session and the review of the eligibility paperwork the SS Program Coordinator …show more content…
If the client is approved by the DCSR then the SS Program Coordinator will call the client and offer them entry into the program. The client will then come back in and meet with the SS Program Coordinator to complete the program participation agreement form. The SS Program Coordinator and the client will also complete the initial Bridge to Self-Sufficiency assessment at that time. Based on the bridge outcome the client and the SS Program Coordinator will identify which sections the client would like to focus on and the client and the SS Program Coordinator will develop overall goals for the client and action steps for the client to meet those goals. The goals will be long term objectives but the action steps will be tasks that can be completed during the month. The action steps will be steps that will help the client to eventually achieve the larger goal. The client will initially develop 1-3 major goals. The client and the SS Program Coordinator will meet on a bi-weekly basis for case management sessions the next 2
