...Patient Safety defines health errors as “the disappointment of a designed action to be finished as planned or the use of an incorrect plan to attain an aim(Longo, D., Hewitt, J. Checkup errors that reason damage or hurt are occasionally called avoidable unpleasant actions that are the damage is a reflection to be due to a therapeutic intervention, not principal patient circumstances. Errors consequential in severe injury or death are considered “sentinel events” by the Joint Commission (Longo, D., Hewitt, J. E., Ge, B., & Schubert, S., 2007). Helping people was always a goal of mine, and I have been able to fulfill that goal and set new ones in that realm within the world of nursing. My past business experience has provided me with the knowledge to not only provide safe, quality care for my patients, but to relate to families and hospital staff in a clear and effective manner that improves outcomes for my patients. I have also participated directly in reducing our blood stream infection rates substantially, and in eliminating waste and duplicate supplies, which have saved my unit hundreds of thousands of dollars. Evidence Based Practice

...second most common nosocomial infection. HAP consists of approximately fifteen percent of all hospital-acquired infections and occurs in five to fifteen percent of every 1,000 admissions. Critical care nurses are presented with unique challenges when caring for patients who are at risk for developing VAP. Strategies must be developed using evidence-based practice and incorporated into daily care in order to prevent VAP in patients receiving mechanical ventilation. ICU Doctors, Nurse Educators and the DON on the ICU unit are currently responsible for our current oral care practice. Our hospital is currently using (OAG), which stands for Eiders’ Oral Assessment Guide, it measures a patient’s oral health status and the frequency of oral hygiene. This tool measures oral health by using 8 different categories: swallow, lips, tongue, saliva, mucous membranes, gingiva, teeth, and voice. They are graded 1 (best), 2, or 3 (worst). Normal findings would be a score of 8, whereas 24 would be the total worst score in all 8 categories. I will now explain how using evidence based practice and OAG can improve the oral health of patients on ventilators. The conclusion will be supported by the research that I have obtained in this research project, which is decreasing the risk of pneumonia on patients who are on ventilators. The provided back ground information was relevant and direct. Hospitals could save millions of dollars by preventing nosocomial infections and using evidence based practice...

: Describe Compare and Contrast the Traditional Abstinence Model of Addiction Treatment with the Modern Harm Reduction Model in the Context of Counselling a Heroin Using Client Who Is Hiv or Hep-C Positive

...Title of assignment: Describe compare and contrast the traditional abstinence model of addiction treatment with the modern harm reduction model in the context of counselling a heroin using client who is HIV or Hep-C positive I declare that this assignment is entirely my own work and that all sources are credited. Introduction: In this essay I am going to first define abstinence, harm reduction, heroin and HIV. Then i’m going to compare and contrast the abstinence model versus the harm reduction model in relation to a person who uses heroin and has also HIV under the counselling context. Definitions: The Abstinence Model: The term 12 step applies to a number of self-help groups which have grown from the original alcoholics anonymous movement. 12-step groups are fellowships of men and women who meet regularly to support each other in their efforts to achieve and maintain abstinence. Groups are run by the members themselves who are not professional counsellors. They have a spiritual foundation are not affiliated with any organised religion. (McMurran, 1994) For the purposes of this essay groups like NA(Narcotics Anonymous) would represent a traditional abstinence group and the Minnesota Model would represent a counselling approach in relation to addiction counselling. The Harm Reduction Model: The idea of harm reduction first emerged explicitly in Dutch drug policy during the 1970s and 1980s from concern about the social integration of people who use drugs into...

...ISTQB Advanced CTAL Exam – S tudy Guide (Part 1) Q. 1: What is Configuration management? Software configuration management encompasses the disciplines and techniques of initiating, evaluating, and controlling change to software products during and after the development process. It emphasizes the importance of configuration control in managing software production. Configuration management is an integral part of the software development process across all phases of the life cycle. It functions as a controlling discipline, enabling changes to be made to existing documentation and products in such a way as not to destroy the integrity of the software. Since configuration management extends over the life of the product, and since tools, techniques, and standards exist solely aimed at its proper execution, configuration management can stand alone as a module within a graduate curriculum. > Q. 2: What are the Requirements for the Success of Configuration Management? The key requirement for success of configuration management is the commitment of all levels of management to enforcing its use throughout the project lifetime. Configuration management, like other parts of software engineering perceived as being tedious, may require some coercion for success. A further requirement is the availability of a clearly stated configuration management plan. > Q. 3: How can we say that Configuration Management is a Cost Saving Tool?
