A. Small Business
Klinik “DK” Klinik “DK” adalah klinik tingkat pratama yang memberikan jasa pelayanan kesehatan meliputi pelayanan promotif, preventif & kuratif kepada pasien ( umum & gigi ) berbasis asuransi ( BPJS, In-Health, Jamkesda & perusahaan ) dan pasien pribadi/swasta untuk tingkat layanan dasar.
Pada awalnya, Klinik “DK” adalah sebuah praktek dokter umum mandiri, dari dua orang dokter suami dan istri, mereka adalah dr I. Eko Setiawan dan dr. Wisnu Murti Yani. Terletak Jl. Ring Road Utara 17A, Karangnangka RT 7 RW 13, Maguwoharjo, Depok, Sleman no telp (0274) 4333679, berada di pinggir jalan arteri, dan mudah diakses masyarakat sehingga cepat di kenal dan dalam waktu singkat tumbuh dengan pesat, sehingga pada 28 Maret 2010, akhirnya praktek umum mandiri tersebut berubah menjadi Klinik Pratama Rawat Jalan dengan nama : KLINIK “DK”, nama “DK” merupakan singkatan dari “Dokter Keluarga”.
Untuk menganalisa sistem bisnis yang berjalan di Klinik “DK” kami menggunakan pendekatan “6M” ( Method, Man, Money, Machine, Material & Market ).

METHODE Pengertian ‘methode’ dalam hal ini adalah meliputi cara kerja & ruang lingkup yang menyertai dalam berjalannya sistem bisnis di Klinik “DK”, meliputi : Visi, misi, bisnis utamanya ( main business ), ruang lingkup serta bentuk & standar pelayanannya.
Bisnis Utama
Visi : “ Menjadi klinik dokter keluarga terbaik se Sleman dan menjadi pelopor pengelolaan kesehatan dasar masyarakat di DIY yang mencakup upaya kesehatan Promotif, Preventif & Kuratif ”
Misi : “ Mengembangkan sistem pelayanan kesehatan berbasis dokter keluarga dengan melibatkan seluruh anggota keluarga sebagai agen kesehatan demi terwujudnya program Indonesia Sehat “ Adapun bisnis utamanya adalah memberikan jasa pelayanan kesehatan meliputi pelayanan promotif, preventif & kuratif kepada pasien ( umum & gigi ) berbasis asuransi ( BPJS, In-Health, Jamkesda & perusahaan ) dan pasien pribadi/swasta untuk tingkat layanan dasar. Provit yang didapat dari Klinik “DK” agak berbeda dengan klinik-klinik konvensional yang lain karena sistem pelayanannya berbasis asuransi, maka sangat ditentukan oleh kemampuannya dalam hal memberikan pelayanan kesehatan yang prima dan bisa dipercaya pasien maupun vendor ( pengelola asuransi seperti BPJS, In-Health ) sehingga memberi dampak peningkatan jumlah kepesertaan pasien/asuransi sekaligus juga ditentukan oleh tingkat kemampuannya dalam menekan angka kesakitan seluruh pasien yang menjadi anggotanya. Untuk itu selalu ditekankan dalam memberikan pelayanan kesehatan tidak hanya fokus pada kuratif/pengobatan tetapi fokus juga dalam upaya pencegahan penyakit melalui upaya promotif & preventif.
Dalam sistem asuransi dikenal ada 2 sistem pembayarannya : (1) Sistem Kapitasi : adalah sistem pembayaran asuransi kesehatan dengan cara membayar sejumlah dana resiko kesakitan yang dibayar di depan perbulan sesuai jumlah kepesertaan anggotanya, contoh; BPJS, In-Health (2) Sistem Reimbas : adalah sistem pembayaran asuransi kesehatan yang dibayarkan pada periode bulan berikutnya sesuai dengan biaya yang dikeluarkan oleh peserta asuransi tiap bulannya, istilah mudahnya klinik tsb menalangi dulu biaya pemeriksaan yang nantinya akan ditagihkan dan dibayar pada periode bulan berikutnya, contoh; asuransi perusahaan.
Maka strategi pendekatannya untuk memperoleh provit jelas berbeda, untuk yang sistem kapitasi pada prinsipnya makin kecil prosentase angka kesakitan per bulan maka makin banyak provit yang di dapatkan, oleh karena itu klinik “DK” tdk hanya fokus untuk kuratif tapi justru lebih fokus dalam upaya promotif dan preventif ( nanti akan lebih detail pada uraian berikutnya ). Untuk yang sistem reimbas ditentukan oleh kesepakatan awal dengan vendor tentang besaran jasa pelayanan kesehatan, tentu saja pihak vendor yang lebih berperan dalam menentukan pilihan klinik mana yang dijadikan rujukan kerjasama, secara umum biasanya yang dipertimbangkan adalah besarnya klaim perpasien, lokasi, referensi pihak lain (peserta, vendor yg sejenis ) dan kelengkapan cakupan pelayanan, oleh karena itu strategi yang ditempuh oleh klinik “DK” berupa memberikan besaran biaya yang rasional dan memberikan fasilitas yang lengkap ( umum, gigi dan apotik ) karena beberapa klinik kompetitor hanya ada umum saja, gigi saja atau ada umum & gigi tetapi tidak ada apotik.
Karena pendapatan terbesar klinik “DK” adalah dari pihak asuransi yang menggunakan sistem kapitasi maka klinik “DK” menggunakan strategi pendekatan Promosi, Preventif & Kuratif terhadap para peserta asuransi di bawah layanan klinik “DK”. Beberapa contoh pelayananan jasa kesehatan dalam bentuk promotif & preventif : membentuk dan membina kelompok lansia yang bernama “ Prolanis Maguwo Sehat “ yang berisi kegiatan membina para lansia yang menderita penyakit kronis ( DM, Hipertensi ) unutk tetat memiliki kualitas hidup yang sehat di usia senja dengan cara rutin periksa laboratorium dan senam untuk lansia ( senam DM, senam jantung sehat ), penyuluhan rutin ke komunitas tertentu ( PWRI ) ttg tema kesehatan lansia. Beberapa contoh kegiatan yang bersifat promotif & perventif.

JENIS PELAYANAN
Jenis Pelayanan yang dapat diakses di Klinik ”Dokter Keluarga” adalah: 1. Pelayanan Pemeriksaan Dokter Umum 2. Pelayanan Pemeriksaan Dokter Gigi 3. Pelayanan Tindakan Medis Dokter Gigi: pencabutan gigi, tambal glass ionomer, tambal sinar, perawatan saraf, pelayanan gigi palsu, dan operasi oklusi) 4. Tindakan Medis Dokter Umum 5. Tindakan Bedah Minor 6. Pelayanan Keluarga Berencana (KB): Suntik, Pil, IUD, Implan/susuk KB 7. Pelayanan Imunisasi BAYI, BATITA, dan HAJI/UMROH 8. Pelayanan Obat Klinik oleh Apoteker 9. Laboratorium Sederhana (Rapid Tes Kolesterol, Gula Darah, Asam Urat) 10. Laboratorium Lengkap (Bekerjasama dengan Pramita Lab.) 11. Fisioterapi Sederhana (Diatermi) 12. Serta Pelayanan Kegawatdaruratan Terbatas 13. APOTEK ( sedang dalam proses perijinan)

M A N Pengertian “Man” dalam hal ini adalah meliputi sumber daya manusia, struktur organisasi dan peningkatan kompetensi profesi ( Dokter, dokter gigi, apoteker, perawat & bidan ).
Sumber Daya Manusia atau ketenagaan di Klinik DK adalah sebagai berikut : * 4 Orang Dokter Umum Tetap/Utama dengan SIP di Klinik “DK” * 3 Orang Dokter Pengganti * 1 Dokter Gigi; * 1 Apoteker Penanggung Jawab, (dan 1 Apoteker Pendamping utk Calon APOTEK) * 3 Perawat; * 3 Bidan; * 1 Perawat Gigi; * 2 Staf Administrasi; dan * 2 Tenaga Kebersihan

M O N E Y Seperti yang diuraikan di atas bahwa klinik “DK” sebagian besar pendapatannya dari sistem kapitasi asuransi ( BPJS & In Health ) dan ada beberapa tambahan dari sistem reimbas ( Jamkesda, PLN ) dan dari pribadi/swasta ( bayar langsung ). Sebagai gambaran untuk yang sistem kapitasi setiap bulannya klinik “DK” menerima dana kapitasi sesuai jumlah peserta asuransi yang terdaftar sebagai member klinik “DK”, sampai saat ini yang menjadi member klinik “DK” ada 7000 anggota untuk BPJS dan 400 anggota untuk in Health dengan ketentuan, besaran kapitasi per anggota untuk BPJS sebesar 10.000,- untuk in Health sebesar 20.000,-. Hal itu sudah termasuk pelayanan periksa dokter umum & gigi serta obat-obat dan bahan-bahan medis yang diperlukan. Sehingga agar klinik “DK” bisa mendapatkan “provit” harus mengupayakan tingkat kesakitan perbulan di bawah 25%, untuk itulah strategi penanganan pasien lebih dipriorotaskan dalam bentuk promosi & perventif.

M A C H I N E Dalam hal ini yang dimaksud dengan “machine” terkait dengan Klinik “DK” adalah suatu alat atau bahan yang bersifat memudahkan proses bisnis yang dilakukan oleh klinik tsb di bidang jasa kesehatan. Adapun yang di masukkan dalam kategori ini adalah : (1)Aset tak bergerak ( area parkir, ruang periksa, ruang tunggu ), (2)alat penunjang medis ( dental unit, tensimeter ) (3)alat penunjang non medis ( komputer )

Obat – obatan yang di pakai adalah untuk keperluan dokter umum dan gigi yang terdaftar DPHO.

M A R K E T Secara umum target pasar dari klinik “DK” adalah seluruh penduduk Sleman terutama para peserta asuransi (BPJS, In Health, Jamkesda & Asuransi lain). Meskipun begitu klinik “DK” melihat bahwa pasar potensial adalah daerah-daerah terdekat dari klinik tsb, yaitu : Maguwoharjo (domisili klinik), Purwomartani, Catur Tungggal, Condongcatur, Sinduadi & Minomartani. Berdasarkan data sensus 2011 tercatat bahwa jumlah penduduk Sleman ada 1.125.369 (perempuan : 566.067, laki-laki : 559.302), dengan asumsi target 1% saja terdapat angka 11.254, sedangkan jumlah pasien klinik “DK” baru mencapai 7.400, maka masih besar sekali potensi pasar yang masih bisa digarap (+/- 4.000) dengan data sensus 2011 dan di 2015 pastilah terjadi peningkatan jumlah penduduk sehingga target pasar yang belum terjamah masih lebih besar lagi. Dalam jasa pelayanan kesehatan tidak dibedakan jenis kelamin & melayani pasien sejak lahir sampai akhir hayat, otomatis makin memperkuat potensi pasar yang sudah ada.
