Cardiovascular Prevention Program

Reducing the risk factors related to chronic diseases is difficult, especially in the population that has heightened risk factors. Risks factors such as hypertension, hyperlipidemia, high glucose are ones of the most common risks factors that could lead to chronic diseases such as cardiovascular diseases and diabetes mellitus. CVD in particular, is the leading cause of death for adults 65 and over (and second for adults 45-64) (Kaiser 2013). The table below shows the percentage of adults that has been told by the doctor that have a type of CVD:

There are over hundreds of these risk factors, which is caused by biological or environmental. For people who are having difficulty in accessing health care and struggling financially, it is difficult for them to look after their health, thus not being able to maintain their risk factor. It is important that we need to intervene and help the patients and …show more content…
For example, the most common risk factor is hypertension, which is associated with various complications such as cardiovascular diseases, kidney problems, and several more. Risks factors influence by biological, environmental, and social determinants. Most of the patients in the clinic have heightened risk factors in considering of various reasons such as lack of health care access, financial struggles, lack of access to the resources, and other environmental and social factors. Prevention programs exist in many institutions, especially in hospitals. An ideal prevention program has the ability to provide professional care, education, research, and clinical management. They also have various resources that can provide help depending on the circumstances and needs such as dieticians, pharmacists, physicians, psychologists, social welfare, and vocational
