According the CBS News article titled Hospital medication error kills patient in Oregon (2014) a patient died as the result of the wrong medication been given at the St. Charles Health System Hospital in Bend, Oregon. The patient was 65-year-old Loretta Macpherson, who was supposed to receive an anti-seizure medication fosphenytoin. Though she received instead a paralyzing agent, rocuronium, used routinely in surgery that caused her to go into cardiac arrest and suffer brain damage. She was put on life support but was removed, and Macpherson died. The reason Loretta Macpherson sought hospital care was due to medication dosage questions pertaining to her recent brain surgery.
