Research into the implementation of health care information systems spans more than thirty years at a cost of millions of dollars (Zheng, McGrath, Hamilton, Tanner, White, Pohl, 2009). In spite of those costly efforts, patient records continue to be primarily paper-based. The Institute of Medicine (IOM) (1991) of the National Academy of Sciences recognized the magnitude of the problems associated with paper medical records systems and called for the adoption of computer-based patient records (CPR) or electronic medical records (EMR) as the standard for all patient records by the year 2001. EMR systems have been shown to have value in patient care; they are not widely used by clinicians in community-based practice. Although there are barriers to the productive use of EMR systems in primary care and there are situations in which such systems have failed, there are early adopters of this technology who have successfully implemented the systems and made them an integral part of their organizations. Jeremy Bentham thought the very expression to be a ‘paradoxical and unmeaning epithet’.[1] In his view, social reform required ‘fixed and accurate’ laws—laws that ‘[w]e see, we hear, we touch; in short we handle…’[2] This empirical concept of law follows from the ideal of law as something produced by conscious acts of sovereign will. Central to this concept are, to use H.L.A. Hart’s expression, ‘rules of change’ that permit societies to escape the confines of ‘primitive’ custom.[3] The progressive march of legal theory, it may be said, has been away from medieval notions of law as customs practiced time out of mind or as the immutable dictates of natural law, away from the fiction that judges discover rather than make law, and towards modern notions of law as creative political acts recorded in writing. Earlier studies on the effects of building air intakes have been limited to relatively simple situations, unable to treat the complex envelope of most buildings and building groups. Computational Fluid Dynamics (CFD) is a tool that assists in modeling the airflow and dispersion of pollutants among complex urban geometries on the scale of a section of a building’s exterior up to several city blocks. This tool allows more accurate predictions of impacts over a range of meteorological scenarios and alternative building designs and placements relative to roadways and other pollutant sources. The steps in a CFD application are presented including geometry and mesh creation, simulation of meteorological conditions, handling of pollutant sources, and post-processing visualization. Evidence for potential health hazards accumulated in parallel by epidemiologic investigations has raised controversies about the appropriate interpretation and the degree of bias and confounding responsible for reduced or increased risk estimates. data sources: Overall, I identified 33 epidemiologic studies in the peer-reviewed literature, most of which (25) were about brain tumors. Two groups have collected data for ≥ 10 years of mobile phone use: Hardell and colleagues from Sweden and the Interphone group, an international consortium from 13 countries coordinated by the International Agency for Research on Cancer. data synthesis: Combined odds ratios (95% confidence intervals) from these studies for glioma, acoustic neuroma, and meningioma were 1.5 (1.2–1.8); 1.3 (0.95–1.9); and 1.1 (0.8–1.4), respectively. conclusions: Methodologic considerations revealed that three important conditions for epidemiologic studies to detect an increased risk are not met: a) no evidence-based exposure metric is available; b) the observed duration of mobile phone use is generally still too low; c) no evidence-based selection of end points among the grossly different types of neoplasias is possible because of lack of etiologic hypotheses.
