As required for completion of the OCAF Rent Adjustment enclosed are three original copies of the executed Rent Schedule, HUD form 92458.

Please note that on the Rent Schedule under Services / Facilities the swimming pool has not been checked since the pool will be closed permanently. In regard to the pool closing I have attached a copy of the letter we received from HUD dated 6-23-2015, approving the closing of the pool.
