On 8/25/17 WC met with tenant alt the SUS cookout at Prospect Park. Nancy was dressed appropriately and groomed. She appeared to be mentally stable. She was eating, laughing and socializing with other tenants and SUS staff.
Wellness worker asked Nancy how he was doing and she responded that she was “doing fine.” Tenant was very social with other tenants and appeared to be having a good time. Mrs. Roman reported to WC that she has been going to her day program regularly and has been eating well. She reports sees her psychiatrist and therapist at her day program.
Client states continues to have difficulty sleeping and that she is seeing her doctor to address the issue. She states that she has not had any recent conformations with her
