Did you find chart helpful? Can you think of anything you would like to know that was not in the chart? I did find the chart very helpful because it explains the patient’s problems extremely well. If the practitioner has not seen the patient before, the chart is perfect to learn the basics of what is wrong. I did think the chart was harder to read than what we viewed in class. I do think the precautions could have been expanded as the only thing I could find was the low fall risk aspect.
Did you find the information easy to locate? Explain your reasoning or how you would improve upon the chart if you did not find it easy to locate. A lot of the information that is in the bulleted list was not listed directly in the chart. I thought the chart was more like a scavenger hunt as I had to pull information from different categories to answer one question. I liked how the vital information was added to every page of the evaluation, and each page of the therapy sessions. I would hope the co-worker would tell me what the long-term and short-term goals were for the patient. This is what I thought would be one of the most important pieces of information because the occupational therapy assistant has to know the goals before they begin working with the patient. Another aspect that would be important to know is if the patient had any precautions that would need to be known about before treatment.
