The claimant claimed before her alleged trip and fell at work on 8/6/2015; the claimant claimed she did not have any diseases of the neuromuscular system, injuries, metabolic diseases associated with muscle weakness to her either of her legs. She stated that she does not suffer or has ever been diagnosed with any inflammation in the knee joints, caused by osteoarthritis or rheumatoid arthritis.
On the claimants’ alleged DOI: 8/6/2015, the claimant stated before she took her lunch break at 12:00 p.m. she was at work when she pass walking past the conference room near the City Clerks Office when she heard and recognized her co-worker, Mr. Fabiano Villegas call out her name. She said as she turned to the left to turn towards Mr. Villegas, she “pivoted on her left foot” and felt as she twisted her left ankle when she fell on her “left hip.” …show more content…
