Declaration of Misuse of ID
This is a TCIC/NCIC File. In accordance with TX HOUSE BILL 153, effective September 1, 1999, a procedure was established to prevent the fraudulent use of an individual’s identification in circumstances affecting proper law enforcement and to assist law enforcement agencies in verifying that an individual’s ID has been used by another person. NCIC 2000 began allowing record entry April, 2005:
CRITERIA FOR ENTRY:
1) An individual must contact the Local Sheriff’s Office to make a report that his/her ID has been stolen.
2) The Sheriff’s Office is responsible for taking the individual’s fingerprints and submitting them to the department (DPS) for verification. After fingerprint verification, notification of the person’s identity is sent from DPS (via teletype) to the Sheriff’s Office.

The Declaration must include the individual’s name, race, sex, date of birth, height, weight, hair color, and date of report, social security number, driver’s license number, originating agency case number (OCA), Agency ORI, and a unique password.
