...Kyle Thompson – 05970640 Management of discharge Management skills in adult nursing UZTR6D4-40-2 05970640 The NMC Code (2008) charges nurses to protect confidential information, and to only use it for the purposes given – for their treatment. Therefore all patients and events mentioned in this essay are inspired by real patients and events, but names, locations, dates and other details have been altered or obscured to make identification impossible. Following the introduction of the knowledge and skills framework (DH 2004a) and emphasis on quality of health care and patient centred, interprofessional, health and social care (DH 2000; Leathard 2003; Thompson et al. 2002) health care professionals and students will need to be able to demonstrate the quality of our care and team working abilities. There is a connection between practice and thinking about practice – action and reflection are interdependent; they need one another. Reflection may be triggered by an awareness of a gap between theory and practice, a difference between what ‘should be’ and ‘what is’ (Sullivan & Decker 2005). Our actions and the quality of our care are improved by reflection-on-action, by making sense of what we have experienced, and thinking about how we might act differently in the future (Lillyman & Ghaye 2000). Reflection has a rôle in maintaining one's personal portfolio and maintaining competency and continuing professional development. Support and supervision from managers, who already have a responsibility...

...Successful discharge planning is not something that should be initiated when the patient is ready to go home, but should be discussed prior to the surgery being performed if possible. The total hip replacement that Mr. Trosack is recovering from was not a planned surgery so case management should have begun working on this once he was admitted to the floor postoperatively. Healthcare Issues and Their Importance There are numerous healthcare issues that must be considered in discharge planning for Mr. Trosack to ensure that medical, social and functional issues related to his recovery have been evaluated. Safety is a very important healthcare issue that must be addressed prior to discharge. As with most patients over the age of 65, Mr. Trosack is at high risk for a fall for various reasons. 33% of this age group experience one fall per year and commonly involve falling down stairs or steps (Heasley, Buckley, Scally, Twigg, & Elliott, 2005). He lives on the second floor of an apartment building that has no elevator so he must maneuver a long flight of steps to enter or leave his apartment. In addition to this, he has a small apartment that is cluttered with WWII .memorabilia and furniture which increase his chances of tripping and falling. Mr. Trosack has recently been diagnosed with hypertension and noninsulin dependent diabetes which will present another healthcare issue and result in a lifestyle change for him. Previously, he was not taking any prescription medications and...

...The role that discharge planning plays in the admission of a patient is a key component of any healthcare business that must start upon admission. You may not understand how the term business relates to a hospital setting where their focus is healthcare but do not let yourself develop false beliefs, a hospital is a business. Just like any business it must have a beginning a middle and end where revenue is generated or it cannot continue to function. This is where discharge planning holds it ground. Birjandi and Bragg second that fact by pointing out that one main problem that compounds this inability to capitalize on the use of discharge planning is as I explained previously, "that most senior leadership teams do not understand the complexity...

...PERSONAL NARRATIVE 1 When taking a look at how my life has changed over the past five years I can truly and honestly say that I would never have expected the things that have happened. Back in June 2007 I was separated from my husband and moved into my own apartment. I was in the United States Navy for six and a half years at that time. My divorce was finalized in January 2008 and I deployed to Afghanistan in March 2008, for eight months. After coming home from a long deployment, I had orders to move to Lemoore, California. This area is nothing but farm lands and I did not like that at all. I was born and raised in Southern California and enjoyed the fast-paced life that I was living. It is amazing how things have changed. In July 2011 I was honorably discharged from the United States Navy and I moved back in with my mother and two younger sister. Growing up we lived in a condo in Diamond Bar, California, but that all changed over time. The same time that I got out of the military, myself, my sisters and my mother had to move out of our condo and into a two bedroom apartment. Talk about a huge change in life. This move has not been easy for any of us. There is no privacy and we all have to share the living space. I share a bed with my middle sister and my mother shares a bed with my youngest sister. I never thought that I would be unemployed for this long, but it has been over a year since I got discharged. It seems like nobody is hiring right now...

...Few good man Time from time you may hear stories of abuse by the military justice "system". There are multiple destroyed and financially devastated families out there suffering heartbreak and loss at the hands of an uncaring, unfeeling and unjust process which makes a mockery of our Bill of Rights. Military justice for the majority is prefabricated according to the wishes of the local Commander, and the "trial" or "court-martial" is tantamount to a pre-ordained verdict of GUILTY. How could any court proceeding be considered fair when the "convening authority," by right of title, is given the power to select the judge, the jury and defense and prosecution attorneys? It may go "unsaid," but the implication is very clear - if the convening authority "sees fit" to bring about a court-martial, then the accused can be assumed to be guilty. What's special, or even topical, about A Few Good Men movie is it speaks to how military honor can be so readily suborned by the authoritarian impulse. And second, how the same honest pride - not to mention competence - is necessary to bring such posturing would-be tyrants to justice. The main issue the movie touches upon is military justice. A good marine abides by the following code: 1. Unit 2. Corps 3. God and 4. Country For this code to be at its effective best, it must be governed by truth and justice. What happens when truth and justice fail to be active participants of this code? Justice has been defined in many ways...

...stability. But ceramic caps have a very less ESR and are useful in bypassing high frequency noise but cannot provide the compensation to maintain the loop stability as the dominant zero is at a very high frequency. So LDO’s with ceramic output capacitors have an additional series resistor R comp which performs the functionality of ESR with the C comp to provide the additional zero required for compensation. Load dump occurs when the battery is disconnected when it is being charged by the alternator. A huge voltage spike of 120V is generated and can take up to 400ms to decay. Signal diode arrays can also be used in digital and computer circuits to protect high speed data lines or other input/output parallel ports against electrostatic discharge, (ESD) and voltage transients. By connecting two diodes in series across the supply rails with the data line connected to their junction as shown, any unwanted transients are quickly dissipated. Signed __________________ Date _________________ Abstract Windsor Airmotive Asia Pvt Ltd is the leading repair and overhaul shop for CFM56-3/-5/-7 series turbine engine components. During development of a new repair scheme, to replace the inner inducer and outer inducer of the Forward Inner Nozzle Supports (FINS), a major challenge was encountered at the Electric Discharge Machine (EDM). A trial done on a scraped part revealed that 112 vanes have to be aligned before it can be machined by EDM to the inducer. By introducing a semi-automated machining fixture, the alignment of the vanes will be made easier, on top of increasing the utilization of the EDM by 20%. The aim of this project is to develop a machining fixture for the EDM so that the alignment of 112 vanes will be made easier.By developing this fixture, the company can expect to reduce the scrap rate of FINS. These FINS are often scraped because of the inducer wall rub marks that exceed the limits given in the engine manuals. This will also be beneficial to the customers by helping them to reduce their overhaul cost for the engine rebuild. This will...
