1. This client has several risk factors for esophageal cancer including smoking, age, sex, GERD, alcohol consumption, excess body weight, and Barrett’s esophagus. 2. IBW = 106 + 48 = 154 lbs BMI = 25 kg/m2 %IBW = 165 lbs/154 lbs x 100 = 107% %UBW = (185-165)/185 x 100 = 11% This client has had an 11% weight loss which puts him at risk for complications. Calories: 75 kg x 30 = 2,250 kcal per day Protein: 75 kg x 1.5 g protein/kg = 112.5 g of protein per day 3. The chemotherapeutic agents he will be receiving are likely to cause nausea, vomiting, diarrhea and poor appetite. Receiving radiation is likely to cause inflammation of the mucous of the esophagus. 4. I do not think this client is going to be capable of sustaining ample nutrition while undergoing radiation and taking the prescribed medications. However, the delivery of nutrients by tube feeding can cause ethical dilemmas in cases where the effectiveness of tube feeding diminishes and medical complications increase. The decision to tube feed is often influenced by regional and cultural preferences, as well as the high cost of providing mealtime assistance. The effectiveness and appropriateness of tube feeding has been the subject of much debate as it applies to those with severe cognitive impairments and those who are in a persistent vegetative state (PVS). Recent research shows that in these vulnerable populations, tube feeding alone does not necessarily prevent malnutrition and risk of infection or improve functional status and comfort. While advanced directives allow an individual to make decisions about his or her care at the end of life, court cases and religious doctrine examine the individual’s right to autonomous decision making in opposition to preserving the sanctity of life. As long as the outcome of this debate is largely undecided, the process of dying may be prolonged for those who can no longer advocate for themselves. It is used to provide nutrients via the GI tract either alone or as a supplement to oral or parenteral nutrition. - Nasogastric (NG) Tube – is most commonly used for short-term feeding problems. Other means of feeding are; esophagostomy, gastrostomy or jejunostomy. Transpyloric tube placement or placement into the jejunum is used when physiologic condition warrant feeding the pt below the pyloric sphincter. ...To me, compulsive overeating is a disease where a person has no control over their food intake. It's a constant eating, a wanting for a more and more. You can't get enough, sort of like a junkie looking for the next high. That is my relationship with food. And I have no control. People that suffer from Compulsive Overeating have what can be labeled not only as an eating disorder, but also as an addiction and even more so as an illness. There are many reasons why people become addicted to food, many using eating as a way to cope with problems or stress in their lives. Eating can also help them conceal their emotions, to fill the emptiness that they feel inside. Food is used as a narcotic to not deal with their feelings or emotions. The tendency for people with this eating disorder is overweight because of the abnormal eating habits. People that do not suffer from the epidemic can and won’t empathize with the victims because of the common stereotypes, stereotype such as greed, gluttony or lack of disciple. Simple suggestions to a sufferer such as “Just slow down or go on a diet” are equally insulting as telling a person suffering from Anorexia to “eat something”. Not only are the words hurtful, but this disorder will negatively bleed into the body and causes health risks.
