...This interruption results in the narrowing of the arteries and heart tissue ischemia, producing symptoms of pain in the chest (angina pectoris) and in serious cases a heart attack and heart failure, even sudden death. Coronary artery bypass graft (CABG) is a surgery that is provided as a common treatment for this condition. The surgery provides a bypass to the blockage in the artery and commonly using patient’s veins (usually from the leg). As a result of the operation patient’s chest pain substantially reduced and patient’s life elongated as a result of restoration of blood flow to the affected heart tissue. However, sometimes patient’s don’t have vessels with a sufficient quality standard. This could be as a result of another disease or previous operations. In this case artificial blood vessels may be used for the vessel replacement. The demand on the small-caliber grafts produced artificially is enormous and drives scientists to carryout research in this...

...ESSAY - (lateral-) meniscus - posterior horn rupture In sports and orthopedics, a tear of a meniscus is a rupture of one or more fibrocartilage strips in the knee called menisci. Anatomy There are two menisci in the knee. The menisci are C-shaped wedges of fibrocartilage located between the tibial plateau and femoral condyles. While the ends of the thigh bone and the shin bone have a thin covering of soft hyaline cartilage, the menisci are made of tough fibrocartilage and contain 70% type I collagen. They conform to the surfaces of the bones they rest on. One meniscus rests on the medial tibial plateau; this is the medial meniscus. The other meniscus rests on the lateral tibial plateau; this is the lateral meniscus. The larger semilunar medial meniscus is attached more firmly than the loosely fixed, more circular lateral meniscus. The anterior and posterior horns of both menisci are secured to the tibial plateaus. Anteriorly, the transverse ligament connects the 2 menisci; posteriorly, the meniscofemoral ligament helps stabilize the posterior horn of the lateral meniscus to the femoral condyle. The coronary ligaments connect the peripheral meniscal rim loosely to the tibia. Although the lateral collateral ligament (LCL) passes in close proximity, the lateral meniscus has no attachment to this structure.[4] The joint capsule attaches to the entire periphery of each meniscus but adheres more firmly to the medial meniscus. An interruption in the attachment of the joint...

...and erysipelas are often used interchangeably and current usage tends to regard erysipelas as a form of cellulitis rather than a separate entity, making clear distinction difficult (Hay 2004, Kilburn et al 2010). Erysipelas affects the upper dermis and superficial lymphatics, with distinctly raised lesions and clear lines of demarcation between involved and uninvolved tissue. Cellulitis extends more deeply, involving the deeper dermis and subcutaneous fat and lacks the distinctive anatomical features of erysipelas. For the purpose of this essay I will be using the term ‘cellulitis’ to refer to both conditions. This essay will endeavor to discuss the incidence, its clinical presentation, risk factors, differential diagnosis, pathophysiology and classification according to severity of cellulitis with reference to current and relevant literature. The management of cellulitis will not be discussed as it goes beyond the capacity of this essay to accommodate this highly researched area (See appendix IV for empirical antibiotics used to treat common SSTIs). Incidence Skin and soft tissue infections (SSTI) are relatively common, however there seems to be a...

...This essay will be a reflective account of an incident from practice based on Gibbs’ Reflective cycle; I will critically analyse the nature of the incident based on the best available evidence. This reflection will help to identify new skills and knowledge learnt as a result of the incident and the implications for my future nursing practice. Although I am aware many reflective models exist, I have chosen to use Gibbs’s reflective framework as it is a cycle and encourages me to think about what I would do differently next time. This clearly shows Gibbs’ awareness that a nurse will have a similar experience again. This reflection method supports Schon’s (1991 cited Issitt 2003) opinion that reflecting on practice after the event is the most effective way to maintain an improvement as a development can be made. As part of the UKCC Project 2000 pre registration nurses are expected to be able to reflect on practice experiences and reflection can help create holistic and flexible practitioners (Andrews et al 1998). For the purpose of the assignment I will use a pseudonym to protect the client’s identity in accordance with the law as stated by the Department of Health (2003), there is also an obligation stated by the Nursing and Midwifery Council (NMC 2008) to protect client confidentiality. This pseudonym is Sue. The first stage of Gibbs’ model is a description of what happened; whilst on a surgical ward I was caring for a lady post operatively who had a bilateral total mastectomy...

...Adjustment to Health Change or Crisis This essay will discuss a patient that I worked with during year two of my course, who has experienced a health change. I will also explore the psychological, social and physical changes to the patient and how adjustment was used on her lifestyle. Corsini (2002,p 20) defines Adjustment as “ Modifications of attitudes and behaviour to meet the demands of life effectively, such as carrying on constructive interprofessional relations, dealing with stressful problematic situations”. In accordance with the Nursing and Midwifery Councils Code of Conduct (NMC, 2009) to protect my patient’s confidentiality, I will refer to the patient as Margret and her husband as Charles. Margret is a 62-year-old lady who is a recently retired shop owner. She has been married for 40 years to Charles. Margret lives with her husband in a 2- bed bungalow. Margret and Charles have three sons. The sons live abroad but Margret has regular contact with them via the Internet and telephone. Margret has no other close family around her. Margret is an active lady and enjoys walking, swimming and other light exercise. Margret had attended a Doctor’s appointment at her local General Practice (G.P.) as she had not attended a check up for well over 30 years. Margret has no other medical problems and has reported to feel fit and well, with no concerns. The Doctor decided to perform a full health check and take a urine sample. The urine sample had detected glucose. Following...

...which can mimic symptoms (See appendix one). I therefore wanted to analyse the available literature in order to be able to competently and safely recognise the condition in the urgent unscheduled care environment. Background Petruzelli (1991) states that Bell’s palsy is an acute paralysis of the facial nerve first described by the Scottish surgeon and anatomist, Sir Charles Bell . Niparko (1993) elaborates that it is a generally unilateral paralysis or weakness of facial musculature consistent with facial nerve damage and dysfunction. The anatomy of the facial nerve can be found in appendix two. Pietersen (2002) states that the cause is unknown, however, whilst the exact aetiology of Bell’s palsy is still debated, viral infections, vascular ischaemia, autoimmune inflammatory disorders and heredity have been postulated as causative. (Adour 1982, Burgess 1984, Lorber 1996). Murakami et al (1996) proposed that reactivation of herpes simplex virus in the geniculate ganglia was causative. A herpes simplex cause is corroborated by Stjernquist-Desatnik, Skoog, and Aurelius (2006). Whilst the exact aetiology is disputed, a greater consensus is reached when considering the pathophysiology. Tiemstra and Khatkhate (2007) state that Bell’s palsy is thought to be caused by inflammation at the geniculate ganglion. This appears corroborated by Fisch and Esslen (1972) who noted oedematous swelling proximal to the...

It should not be thought that Ethics merely relates to the “Life and Death” issues in our Professional life – Abortion, Contraception, Euthanasia and the like. Ethical issues affect some part of almost every consultation, even if the ethical issue is something more mundane like obtaining adequate consent for an examination or respecting a patient’s dignity. Indeed, it could be argued that the Consultation skills that we foster so assiduously are actually Ethical skills – and that we need to know the patient’s “Ideas, Concerns and Expectations” in order to respect his Autonomy as well as in order to improve the outcome of the Consultation. In the 1998/99 academic year, I was appointed the deanery’s Medical Ethics fellow with a bursary from the MDU. I developed an approach to the teaching of GP ethics based on two half day sessions, which...

...McKinsey Global Institute May 2013 Disruptive technologies: Advances that will transform life, business, and the global economy The McKinsey Global Institute The McKinsey Global Institute (MGI), the business and economics research arm of McKinsey & Company, was established in 1990 to develop a deeper understanding of the evolving global economy. Our goal is to provide leaders in the commercial, public, and social sectors with the facts and insights on which to base management and policy decisions. MGI research combines the disciplines of economics and management, employing the analytical tools of economics with the insights of business leaders. Our “micro-to-macro” methodology examines microeconomic industry trends to better understand the broad macroeconomic forces affecting business strategy and public policy. MGI’s in-depth reports have covered more than 20 countries and 30 industries. Current research focuses on four themes: productivity and growth, the evolution of global financial markets, the economic impact of technology and innovation, and urbanization. Recent reports have assessed job creation, resource productivity, cities of the future, and the impact of the Internet. MGI is led by McKinsey & Company directors Richard Dobbs and James Manyika. Yougang Chen, Michael Chui, Susan Lund, and Jaana Remes serve as MGI principals. Project teams are led by a group of senior fellows and include consultants from McKinsey’s offices around the world. These teams draw...
