...Family Health Evaluation A Family Health Evaluation utilizing Marjory Gordon’s Eleven Functional Health Patterns Grand Canyon University May 18, 2014 Family Health Evaluation In 1987, Marjorie Gordon developed a list of Functional Health Patterns that could be used as a guide to assess a family’s health patterns, which could be developed into a data base for that particular family. This data base was projected to help in setting goals for the future health of the family. This paper will discuss the healthy and unhealthy behaviors of the Kessay Family, which are all registered members of the White Mountain Apache Tribe in Whiteriver, Arizona. This family assessment will also discuss what was talked about in trying to develop goals for the improved health for the family. The Kessay family is a traditional Apache family, which includes the mother, dad, and 2 children. The mom is 40 years old, the dad is 45 years old, , and the girls are 14 and 16 years old. The assessment was conducted by asking up to 3 open-ended questions, based on the 11 Functional Health Patterns. Cultural taboos were observed in asking these questions. Some were answered, some were not. Some were answered by very short answers, indicating embarrassment or unwillingness to answer such a rude question. (according to Apache traditions). This writer did the best she could with what was given to her. Health Perception and Health Management: Data collection is focused on the patients current...

...Health Education Plan NUR/405 October 10, 2011 Health Education Plan Educating the public is a role every nurse must participate in to promote, maintain, and restore health among a community. To accomplish such requires community members receive and have a practical understanding of health-related information (Stanhope & Lancaster, 2008). One method to accomplish such is to develop a health educational plan. The purpose of this paper is to create a health education plan based upon information collected from a windshield survey and Friedman family assessment conducted by a nurse in the community of Palmdale, California. Areas that will be discussed include, a description of the educational need based upon assessment findings, educational goals and objectives, educational methods that will be used to implement the health educational plan, and the methods applied to evaluate the educator and process used in the development and implementation of the health education plan. Identification of a Health Educational Need In conducting a windshield survey and Friedman family assessment, the nurse identified multiple educational needs. The priority educational need in common and identified among both the family and the community of Palmdale, California is the need for education on the maintenance of chronic illnesses. Determining what chronic illness to focus on in terms of development of the health educational plan however, presented more of a challenge. When observing...

...The PRECEDE-PROCEED model is a medical model that moves away from the typical issue of disease treatment and focuses on health promotion. Developed by Lawrence W. Green, this model has been applied in many Western countries in the improvement of health. PRECEDE (Predisposing, Reinforcing, and Enabling Constructs in Educational Diagnosis and Evaluation) is the part of the model that helps in the development of various public health initiatives and programs. PROCEED (Policy, Regulatory, and Organizational Constructs in Educational and Environmental Development), on the other hand, serves as a guide to the effective implementation of the various programs that are created using PRECEDE. With regards to using the PRECEDE-PROCEED model in designing an educational program aimed at promoting health for families with school-aged children, the nine phases of the model will be used to produce outstanding results. The first phase is the social assessment phase, where the current social habits and practices of families with school-aged children is evaluated. This assessment helps determine what the existing health practices are, the current quality of life, and the health needs of these families. The second phase involves the epidemiological assessment. According to Webster’s Dictionary, epidemiology is “that branch of medicine which studies the incidence and distribution of disease in a population, and uses such information to find the causes, modes of transmission, and methods for control...

...EVALUATION The sixth phase of the nursing process Is defined as the judgment of the effectiveness of nursing care to meet clients goals based on client’s behavioral responses. Nurses use a variety of skills to judge the effectiveness of nursing care. This skills include knowledge of standards of care, normal client responses, and conceptual models and theories of nursing; ability to monitor the effectiveness of nursing interventions; and the awareness of clinical research . critical appraisal of goal attainment is determined jointly by the nurse and client. The plan of care is the foundation for evaluation. The identified nursing diagnoses, client goals, outcome criteria, and nursing interventions are the guides . through this process, nurses determine the appropriateness, accuracy, and relevance of these nursing care components. Also helps to discover any errors that may have occurred in previous steps. * Figure 13-4 illustrate the relationship of the activities of the evaluation phase to the other phases of the nursing process. *( se debe adjuntar esa imagen a la presentacion ) There are several purposes for carrying evaluation: • To collect subjective and objective data to make judgments about nursing care delivered. • To examine the client’s behavioral responses to nursing interventions. • To compare the client’s behavioral responses with predetermined outcome criteria. • To appraise the extent to which client goals were attained or problems resolved. •...

...The PRECEDE-PROCEED model provides a comprehensive structure for assessing health and quality-of-life needs and for designing, implementing, and evaluating health promotion and other public health programs to meet those needs. PRECEDE (Predisposing, Reinforcing, and Enabling Constructs in Educational Diagnosis and Evaluation) outlines a diagnostic planning process to assist in the development of targeted and focused public health programs. PROCEED (Policy, Regulatory, and Organizational Constructs in Educational and Environmental Development) guides the implementation and evaluation of the programs designed using PROCEED.” (http://www.answers.com) One of the major causes of health related deaths in America today is cardiovascular disease. We live in a society where the majority of the families have parents working full time jobs in order to make ends meet. Parents don’t have time to cook healthy meals and fast food, in a majority of cases, is the answer for quick meals. Because of these factors School Age children often have a poor diet; consumption of soda, candy and fast food is the diet of choice. School Age children are spending more time in front of TV, computers or gaming devices than ever before. Combinations of poor diet and lack of activity is a major factor in childhood obesity. Childhood obesity will be presented as a topic and we will assess the need for this issue in the community. By discussing this topic it will help in the reduction of childhood obesity...

...Scenario Evaluation Plan Christina Bowers HSM/270 April 23, 2015 Kristen Peak The Madison Children’s Hospital mission is to the worldwide leader in improving children’s health by providing the highest quality of health care. To do this they need to be the leading source of research and discovery, educate the next generation of leaders in child health, and enhance the health and well-being of children and families in our local community (Appendix B). With this goal I have chosen the Project-Wide Evaluation Plan. The Project-Wide Evaluation Plan has what the Madison Children’s Hospital needs. A evaluation question, source of information, timeline, method and responsible person (Yuen/Terao, 2003, p.53).The Madison Children’s Hospital goal is to make the children better and do research to figure out how to control the chronic illness. Researchers suspect the unmanaged status of the children’s chronic illnesses is strongly related to this concern. The hospital and schools are very interested in addressing this concern (Appendix B). Biobehavioral studies of the children in the context of family and family-community dynamics are encouraged. The FOA is for ages 8-21, they are put into groups of developmental stages and cognitive disabilities (Appendix B). The children is what matters the most, and finding a cure for their chronic illnesses. Performance measurement is the ongoing monitoring and reporting of program accomplishments and progress toward pre-established goals...

...PURPOSE OF THE STUDY Nursing Students This research study seeks to reach the future nurses, that they maybecome aware of their responsibilities when it comes to Family Planningand also to broaden their knowledge about its impact to women. Pregnant Women The result of this study will serve as a basis for the assessment of thepregnant women’s knowledge about Family Planning in the area of Gastambide, Manila. BACKGROUND OF THE STUDY The study was conducted because of its relevance to maternal health. FamilyPlanning plays a vital role to a mother’s health because it allows mothers to have birthspacing. Birth spacing enables mothers to allot time for recovery during labor anddelivery. Some methods also protect women from sexually transmitted disease as well.We had chosen this study because it is also related in our community immersion topicand we find it very interesting because we know that it can be useful later on in life. Recommendations There should be continuous efforts to improve the family planning program, including aneffort to provide integrated reproductive health. Special attention needs to be given tohelping women deal with side effects. Because of demands on women's time at homeand in the work place, health services should be offered at times (and places)convenient to women; for example, on weekends or after normal business hours.Policy-makers should be concerned about the need for more employment opportunitiesfor women, and should encourage women...

The purpose of this essay is to explore the finding of the evaluation from using the Heritage assessment tool between American, (European decent, white), Asian, and Middle Eastern Indian Cultured families. . The Evaluation tool The Heritage Assessment Tool (HAT) , is a check list used to help gain knowledge of patients culture and beliefs prior to initiating care (Spector 2000) It is important for the nurse to have knowledge of the cultural needs to better treat the patient and make them feel more comfortable. Nurses must apply health education to clients that is culturally sensitive, as well as each culture having their own beliefs, and way of doing things.
