It is estimated that 80 percent of serious adverse events involve ineffective handoff between medical providers (Joint Commission Center, 2013, para. 1). “The majority of avoidable adverse events are due to the lack of effective communication” (Solet, Norvell, Rutan, & Frankel, 2005, p. 1094). Varying styles of communication and interruptions are barriers to the effective handoff and can be a detriment to successful implementation of an effective communication strategy. ...Education Improvement Act (IDEIA; 2004) and the No Child Left Behind Act (2002) require that students with disabilities receive research based practices in the “least restrictive environment.” Because the least restrictive environment is often considered to be the general education classroom, the responsibility to implement research-based practices frequently rests with the classroom teacher. However, during their initial teacher training and certification, many teachers may not have received formal instruction regarding the implementation of current research-based practices. Subsequently, teachers often feel detached from the very research base they are now required to utilize (Ayres, Meyer, Erevelles, & Park-Lee, 1994; Boardman, Arguelles, Vaughn, Hughes, & Klingner, 2005). Interviews with teachers conducted by Boardman and colleagues (2005) suggested that teachers often select classroom instructional practices and behavioral interventions based on four criteria: (a) feasibility within the classroom, (b) perceived appropriateness for particular students, (c) availability of required materials, and (d) availability of required professional development or outside expert support. ...leading cause of hospitalization for persons older than 65 years and often necessitates assistance from family caregivers.1,2 Approximately 40% of older adults are readmitted within 3 months postdischarge, resulting in significant health care costs.2,3 Indeed, more than one million hospitalizations attributed to HF cost the patient and United States health care system over $27 billion a year.1 Hospitalizations related to HF are complicated by the aging process, comorbidities, and psychosocial concerns that affect successful management.2 In a study of patient outcomes post– hospital discharge, Naylor and colleagues4 demonstrated that the use of advanced practice nurses (APNs) was effective in improving outcomes for patients with HF. The feasibility of adapting this approach is limited, however; costs for home visits by an APN are high, a shortage of APNs employed in home care exists, and funding for home care has decreased with the Medicare Prospective Payment System.5 An alternative strategy for promoting self-management of HF is the use of electronic home monitoring (EHM).
