Emergency Procedures and Safety Plan

Effective Date: January, 2016
Purpose: To provide a safe and secure atmosphere for clients
Mission Statement: Providing a safe and fun environment through quality programs while exceeding our customer’s expectations
Approved by: Alfred Desport

Policy/Procedure:
1) Fire In the event of a fire: -Stop all activity and remain quiet. -The supervisor/coach/director present will delegate someone to:
-telephone the emergency services stating name and position, telephone contact number, location, emergency type, unaccounted people, assistance required, and known hazards
-check bathrooms, surrounding areas, and offices
-If evacuation is necessary, proceed as directed to the nearest and safest exit leaving belongings.
-If the fire is large, very smoky or rapidly spreading, evacuate the building immediately, activating the nearest fire alarm to warn other occupants.
-Assess the situation and decide on the safest exit, taking the roll book.
-Move members quickly in an orderly manner to the designated assembly area outside the building.
-Seat members and call roll. Report any missing to the supervisor.
-Do not re-enter the building until directed by Emergency Services.
-Attend to those in need of first aid.
Designated Assembly Areas: Grassy area on west side of parking lot, next to Pep Boys for those in the Gym, Preschool Gym, Lobby, and Office areas. Back retention pond for those in the ASAP and Bounce areas.
2) Accidents All accidents causing bodily injury must be reported. An Accident Report must be filled out completely. Use the back of the form or additional paper, if necessary. Be detailed in your report, and write down all the facts you are able to recall. Reports should be completed within 24 hours of the incident.
3) Serious Injury/Life Threatening These situations call for immediate positive action to prevent the loss of human life or some level of permanent damage to the victim. Examples are: -Severe bleeding -Compound fractures -Head wound -Suspected spinal injury Procedures: Assess the situation. Call for help, and for someone to call 911. Contact the supervisor. He/she will contact parent. Remove other children from group and area. Merge groups if necessary. As soon as the situation is under control, complete and submit an Accident Report. Be sure to get the name of the victim and any witnesses.
4) Minor Injury/Non-Life Threatening These situations may require on site first aid. Examples are: -Cuts -Bruises -Muscle strains Procedures: A non-life threatening injury may become life threatening if not handled properly. Administer first aid if necessary. Clean broken skin with antiseptic and apply bandage. Apply ice for bumps, bruises, sprains. Depending on severity of injury, contact the supervisor. He/she will contact parent. Fill out and submit an Accident Report. Be sure to get the name of the victim and any witnesses.
5) Inclement Weather
These situations require being alert to the weather conditions, and immediate response to move clients to safety. Examples are:
-Severe thunderstorm warning
-Tornado warning
-Flash flood
-Hurricane (in the event of a hurricane, we will be closing the center for evacuation until given all clear to return from Florida Emergency Management System
Procedures: When tornado or severe thunderstorm warnings are issued, gather all patrons and employees, and retreat to the Bounce room, on the lowest level. Have the children sit along the walls, staying quiet. Take flashlights in the event the electricity goes out. For flash flooding, stay on main level, and retreat to the 2nd floor gymnastics room, and again, sit along walls, staying quiet. Phone parents, if possible. Wait for danger to pass, and wait with children until parents come get them. No need for Accident Reports unless someone is hurt in the process.

Waiver and Release of Responsibility

In exchange for my child’s use of equipment and enrollment in fitness, tumbling, and gymnastics classes, I expressly agree that the company and its employees are not liable for damages arising from personal injuries (including death) sustained by my child or children in, on, or about the premises, as result of the use of equipment or facilities, regardless of whether such injuries result in whole or in part from negligence of the company. I accept and assume full responsibility for any and all injuries, damages, and losses of any type.
I agree that my child will comply with all rules imposed by the company regarding the use of facilities and equipment. My child will act in a controlled reasonable manner at all times, and if he/she is in violation of the rules, they will be asked to not come back.
I understand that use of the equipment and facility involves risk, which can result in injury or death.
I understand that the company is not responsible for lost, damaged, or stolen items.

I have read the foregoing waiver and release of liability and voluntarily executed this document with full knowledge of its content.

Gym Safety Guidelines

Policy/Procedures:
Flipping Out Gymnastics Center realizes that being in a gym is a fun time for customers to play and work on skills. We have created guidelines for the customers and staff to follow to help ensure everyone’s safety.
1. There will be no food or drink allowed gym. Snacks and drinks must remain in lobby.
2. One person at a time on all pieces of equipment.
3. Do not run across rod floor or tumble track. Walk around at all times even if no one is on rod floor or tumble track.
4. Watch where you are going at all times.
5. Respect our equipment. Do not throw equipment at each other.
6. No balls or ball type sports in the gym.
7. Put mats back where you found them. Follow posted mat guide.
8. No bare feet, and must have clean socks.

Trampoline Rules:
One person at a time on the trampoline. NO FLIPPING on trampoline unless your coach gives permission. Do not enter the pit when/if the rope is open. Do not sit on springs.

Tumble Track Rules:
Make sure PIT is clear before running down tumble track and jumping in. One person on tumble track at a time. Tumble track is one way only. The direction is toward the PIT.
Rope Swing Rules:
When open, there must be an attendant at the rope swing for the swing to be open. If there is no attendant, the rope will be tied off and closed.
When swinging into pit, let go above pit and get out as soon as possible.

PIT Rules:
Do not bury yourself under the foam blocks.
Do not enter head first or land on your stomach.
Climb out to the nearest wall. Remove all mats from PIT.
Beam/Bar Area Rules:
No running between bar or beam areas.
No jeans or pants with a buckle or snaps, buttons, or zippers. Only wear elastic or drawstring pants.
Rings Rules:
No swinging or spinning on rings.
Bounce Room Rules:
When using the big slide, you must use the mat to slide down, to avoid injury. Only two people at one time.
Only four people in small bounce apparatus at a time.
Only six people maximum in large castle apparatus at one time.
No going behind structures near air pumps.
Only use stair area to climb to top of slides. No climbing on partition in middle.
No balls allowed in Bounce Room.
No rough housing.
No sharp objects.
In the event of a birthday party no one allowed in the room except for invited guests.
Climbing Wall Rules:
Only three climbers maximum climbing at one time.
No jumping from top of wall. Only jump from designated red line.

No customers/clients allowed in gym area without coaches at any time. In the event that you have forgotten something, do not go back in unattended. You must tell the front desk, and they will find a coach or other employee to accompany you back into the gym area.
No one allowed on any piece of equipment unsupervised at any time.

I agree to abide by all of these rules/guidelines while attending Flipping Out Gymnastics Center LLC. In the event that I do not follow these rules, I will be asked to leave and not return.
