A telephone interview was conducted with the head of the family Leonard Brenyah. Within the assessment thirty-three open-ended questions were asked according to Gordon’s 11 functional health patterns. Two-wellness diagnosis were identified and identified within this paper according to the family’s individual need. Values and Health Perception Health perception and values are a component of a family health assessment within Gordon’s Functional Health Patterns (FHP). Its function is to evaluate how a family relates to healthy behaviors and lifestyles that feature preventative measures and promotion of health efforts ("Gordon’s," 2011). His values and health perception are both positive and “humble”. According to Leonard, his family takes a “proactive approach” to management of family health status (Leonard, personal communication, October 8th, 2014). Although there are no specific cultural practices, Leonard participates in church programs that teach health and healthy living and is very active in the Seventh Day Adventist church he and his family attend. Family Health Assessment

...A health assessment is an important tool in formulating a health care plan for an individual or family. Nurses therefore have an important role in health promotion and by using the principles of Gordon’s functional health pattern it can assist in developing a diagnosis and nursing care plan. The functional health patterns serve as a framework that is adaptable to all models of care that can be used as a tool for nursing diagnosis for individuals or communities. It is based on the assumption that health and wellbeing is the result of harmony between the body and the environment (Crisp & Taylor 2009, p. 257). The eleven functional health patterns, was used by the nursing student as the basis for an interview conducted with Daphne Trigg. The aim was to identify the functional and dysfunctional patterns in her everyday life. Daphne Trigg is a sixty seven year old woman who lives in the northern rural area of Adelaide. Collection of basic information and sequences of behaviour were captured and these patterns are also used by health professionals to form client data bases, structure assessments and make diagnoses (Gordon 1994, p. 69). This essay will incorporate discussion of relevant literature and aims to gather ‘objective’ and ‘subjective’ (Crisp & Taylor 2009, p. 260) data as this provides an informative awareness of the interviewee’s overall health. Topics covered are the eleven functional health patterns. They consist of Health Perception–health management patterns...

I have completed a family assessment on the Semanovich family using the 11function health patterns created by Gordon. I assessed the family by asking question from each category of the functional health patterns and will briefly discussion my findings in this paper. Reflective Writing

...Assessment is the first stage of nursing process in which the nurse should carry out a complete and holistic nursing assessment of every patient’s needs, regardless of the reason for the encounter. Usually, an assessment framework, based on a nursing model is used. This assignment attempts to define the points of view of two different frameworks used to collect health assessment data, which are Majory Gordon’s Functional Health Pattern (1994) and The Roy Adaptation Model (2009). The next part would be mentioned about codes of professional, ethical and legal considerations. Finally, I would reflect myself regarding to how to collect and organise patient’s comprehensive health assessment data use these frameworks. The Functional Health Pattern assessment framework is particularly useful in collecting health data to formulate nursing diagnoses (Gordon, 1994, p. 69). Gordon has defined 11 functional health patterns that provide for a holistic client database (p.70). The first pattern is Health perception-health management pattern, is focused on the patient’s perceived level of health and well-being, and how to manage patient’s health. The next pattern is Nutritional –metabolic pattern. This assessment is focused on the pattern of food and fluid intake relative to metabolic need (pp. 80-82). The Elimination pattern is the third pattern. In this pattern, data collection is focused on excretory of the patient. Includes regularity and control of bowel and bladder habits...

Family Health Assessment

...The family is measured the natural and fundamental component of the community. The household members make up the family as a structure. The family includes members of different age group from newborns to elders. The family’s activities and reactions influence the patterns of the family as a whole (Edelman, Kudzma, & Mandle 2014, p. 2014). The ultimate aim of the treatment is to expedite the fitness of the family. Family health valuation defines the assessment of actual and potential problem of individual (Gordan 1994). His wife, age 45, home schools their three children, ages 14, 11, and 5. The two younger children are adopted from Guatemala and China. Heritage Assessment

...Family Health Assessment For this health assessment assignment the system theory was used on a family member from the African American decent as a way of assessing the family as a whole and not an individual. The “Systems theory explains patterns of living among the individuals who make up family systems”( Edelman, 2010, p. 173). The family member was interviewed and open ended family questions that focused on the eleven functional health patterns were asked. The eleven functional health patterns are values, health perception, nutrition, sleep/rest, elimination, activity/exercise, cognitive/perception, self perception, role relationship, sexuality and coping. By asking questions about these eleven health patterns the interviewer began to understand the family members values, health status, health practice and health beliefs and the importance of using the system theory for family assessment. The first question focused on family values. The family expressed the importance of having a personal relationship with God and not just a relationship with a place of worship, as a top value in their life. “Meanings and values provide the vital elements of motivation and energy for family systems”(Edelman, 2010, p. 173). They valued having a relationship with God because if an individual does not have a relationship with God they will not be able to overcome the difficulties that they experience in life because they “process information and energy exchange with the environment...

Family Assesment

...Family Health Assessment Introduction A comprehensive family assessment provides a foundation to promote family health (Edelman & Mandle, 2011). Marjorie Gordon developed 11 functional health patterns. They provide a systematic approach for nurses to use during the assessment process. By using this assessment tool the nurse can collect data about functional health patterns. By identifying certain behaviors through this assessment process the nurse can better tailor an appropriate nursing diagnosis, interventions and plan of care. This family health assessment paper has been organized and prepared in the following fashion. First, the questionnaire has been created which includes the 11 functional health patterns. A diagnosis has been presented in “Diagnosis” section, according to questions generated, using course textbook (Weber & Kelley, 2010) and guidelines for specific learning outcomes and conduct of therapeutic conversations by undergraduates from Holtslander & Smith (2013). The proposed list of questions and its format has also been quantitatively evaluated and compared to other documented experiences of family diagnosis using Gordon's 11 functional health patterns (Nettle et al., 1993; Kriegler & Harton, 1992) and their questionnaires used (Nettle et al., 1993). Finally, a wellness family nursing diagnosis has been inferred, based on materials from Weber & Weber (2010). Discussion The interviewed family consists of three currently active members –...
