...The future of Health Care in the United States is more uncertain than at any other time in the past fifty years. This new uncertainty was created by the U.S.Supreme Court’s decision to receive a challenge to the Affordable Care Act (ACA), known by most as Obamacare. If you missed it, the oral arguments on the case were presented before the Court in an historic three days of hearings March 26-28. These were preceded by many briefs submitted to the Court from a broad spectrum of organizations. All of this information is available in the public domain. A detailed analysis of each day of the hearings may be found at healthaffairs.org.blog. A decision from the Court is expected in late June. There is no certainty about how the Court will rule in the case before it. There are widely divergent speculations which are interesting to read, for sure. This is usual and customary. On the other hand, there is a difference this time. The entire healthcare system is likely to be impacted, many millions of citizens will feel it, and the political races from coast to coast are expected to react regardless of the outcome. There is one certainty. The Justices of the Supreme Court have more power to influence the direction and content of how health care is provided and paid for than at any time in contemporary history with the possible exception of the era when Medicare and Medicaid were enacted in the mid 1960s. The Court’s decision will likely have substantial impact regardless of the decision...

...Discussing the future of nursing theory can bring up a wide outlook of the direction of nursing. Everyone has different views and interests concerning nursing’s future. In this discussion, we will cover some of the opinions of what has driven nursing theory development, what will drive future development in nursing theory, and how might nursing practice change as a result of evolving nursing theory. According to "Nursing Theory" (2011), “The nursing profession has evolved greatly over time, moving from dependence upon total medical direction providing basic care into an independent practice modality with its own nursing theory practice, nursing models, and distinct nursing interventions” (Nursing Theory). The requirements of nursing education have grown greatly. Nurses used to only take care of basic ADL’s of patients. Today, many nurses have more knowledge than the physicians working with these nurses. It is only logical that nurses have their own set of theories and models that guide their nursing practice. Nursing theories increase the general knowledge within nursing through research implementation. Theories provide the basis for nursing practice, helps to generate future knowledge, and indicates in which direction nursing should develop. Nursing theories are an attempt by the nursing profession to continue within the professional boundaries of nursing (Nursing Theory, 2011). Nursing theory has also been driven by the advanced educations available to nurses. Advanced education...

...Public health and health literacy is an unfamiliar topic to many Americans and continues to be overlooked and disregarded in the United States Healthcare system. Healthcare professionals and students in the Healthcare field are not specially trained in this area, which makes it hard to construct a system that targets public health and health literacy. Creating a healthcare system that is more focused on preventative measures may help to alleviate costs in the emergency room, leading to better quality of life and directing our system to prevention rather than solely focusing on treatment. Public health initiatives focus on preventative measures such as colonoscopies, mammograms, screenings for diabetes, immunizations etc. Unfortunately, students...

...organization uses to define its direction and make decisions allocating its resources, whereas, the nursing process is a 5 step method used by nurses to ensure the quality of patient care. These processes require the use of informatics as an instrument to achieve the goals of the organization. Strategic Planning and Nursing Process Strategic planning is used to ensure that both short and long term goals of the organization are in tune with the mission of the organization and to ensure that the goals are achievable. The steps of strategic planning are: identification of goals, assessing external and internal environments, analysis of collected data, identification of potential solutions, choosing next steps, implementation of actions, and ongoing evaluation in the perspective of bench marks (Hebda & Czar, 2013). Informatics is an important part of strategic planning and helps to guide the direction and execute the goals set by the organization. Similar to strategic planning, the nursing process is used to administer patient care in a manner that is goal targeted and methodical. Although the five step process of assessment, diagnosis, planning, implementation and evaluation are similar to the steps of strategic planning there are some differences. Comparison Both strategic planning and the nursing process are scientific methods that focus on the eventual future goals attainment. The strategic planning process focuses on the future direction of the organization in the context...

...two years researching and discovering that many roadblocks in our current health care system restrict nurses from responding to the rapidly aging and changing health care setting, the Institute of Medicine (IOM) along with the Robert Wood Johnson Foundation (RWJF) released their report, “The Future of Nursing: Leading Change, Advancing Health” (Institute of Medicine, 2011). It was determined in the report that our current health care system was selling patients short on accessible, affordable, safe, quality health care. In response to their findings the IOM called on nurses in particular to transform their practices and become leaders in the health care arena. This can be accomplished through a lifetime of continuous education and leadership skills developed into a more patient centered health care system. This would ensure a safer, higher quality, accessible and affordable health care system for all (American Nurses Association, 2010) (Institute of Medicine, 2011). Our aging population needs more primary care, acute care, and long term care while most nurses seem to migrate toward specialty care services. The demand for care continues in specialty areas, but sometimes excludes many services needed by the aging population. If nurses were to continue their education from the time they became licensed until they retired or left the medical community, the sparseness that has been created in our health care system from lack of available resources, may find some relief (Institute...

...Eric Yee 12/06/2014 MHR 452 Dr. Farmer Quality Versus Quantity in Health Care System There is health care reform happening today. According to Journal of Healthcare Finance, “Skyrocketing health care costs are forcing payers to demand delivery efficiencies that preserve and promote quality care” (Scamperle 2014). As the demand for health services, controversies of the direction of the healthcare system stirred. Should healthcare providers focus efforts towards quality or quantity care of patients? There is an emerging trend of efforts leaning towards a quality of healthcare direction as opposed to high volume service. This issue sparked my attention because of initially hearing testimony of mal-practice within healthcare facilities. Friends told me of the staff tending to their own needs before the patients’ needs. Other mal-practice includes negligence and ignoring hospital policies that can make the entire hospital liable for damages. Overall, negligent behaviors lead me to critically analyze the source of healthcare system operations. Briefly describing this issue, healthcare providers used a payment plan called Fee-for-Service which reimburses healthcare providers rewards per patient attended. As precautions for low paying reimbursement transactions from insurance providers, healthcare providers such as doctors write extra medical operations, tests, medicines for patients to purchase. Some doctors order more than one visit to have a cushion of funds to avoid a potential...

...and its Impact on the Future of Nursing Focusing on Nursing Education, Primary Care and Leadership Nursing as we know it has evolved from the days of Florence Nightingale to be part of today’s complex health care system. The demands and the expectations of the patient and the society in general has gone beyond basic good bedside nursing. Although the public trusts nurses to provide good care, they also see nurses as assistants without much say or assistants in the health care field who dress wounds, give injections and so on. In order to change direction of nursing to be able to meet the current demands of the profession and for the other health care professionals to see nurses as equal allies, an overhaul is required. To address this issue, the Robert Wood Johnson Foundation (RWJF) teamed up with the Institute of Medicine (IOM) to find ways to bridge the gap between the nursing we have today and the nursing that can meet future demands. “The Institute of Medicine is an independent non-profit organization that works outside of the government to provide unbiased and authoritative advice to decision makers and the public” (2010). Their goal is to reform the US health care system. To achieve this objective, they focused among other things, on nursing education, leadership role, and practice, particularly primary care. In writing this paper, I will focus on how the IOM intends to improve nursing education, nursing practice, and in particular primary care and leadership. The fact...

...The Community and Financial Role of the Healthcare Manager What is a Community Health Centers? Community health centers are public or non-profit clinic sites located in medically underserved, rural, and urban areas throughout the nation. They receive grants under the Community and Migrant Health Centers Program of the US Department of Health and Human Services to provide primary and preventive care to community residents. Community health centers remove common barriers to care by serving communities that otherwise confront financial, geographic, language/cultural and other barriers, making them different from most private, office-based practices. CHCs are: • Located in high-need areas identified by the federal and state government as having elevated poverty, higher than average infant mortality, and where fewer providers agree to practice; • Open to all residents, regardless of insurance status, and provide reduced cost care based on ability to pay; • Tailor services to fit the special needs and priorities of local communities, and provide services based on the advice of local residents, businesses, churches, and other organizations; and • Offer services that help patients access health care, such as transportation, translation, case management, health education, and chronic disease management. Health centers are required by law to provide: • Basic health services related to family medicine, internal medicine, pediatrics, obstetrics, or gynecology; ...
