2300 hrs. EMS 10 responds to a 24 year-old male who has been shot. He is conscious and breathing. Law enforcement has been dispatched. Pt’s chief complaint is a gunshot wound and shortness of breath. Hx of similar events: Stated that he has previously been “shot twice in the chest”. NKDA. Last oral intake was 2 hours ago. Pt was spinally immobilized and log rolled onto spine board, at this time the Pt’s entire back was assessed. Patient was then placed on stretcher and further secured with seat belts and two side rails up without incident. Pt to be transported to Ben Taub General Hospital due to the nature of the injury. Patient assessment: 24 Year old male, Pt is AAOX4, Pt's condition is unstable. Breathing is irregular, fast, and guarded,
