...Organizational Change Plan -Part 1 Kristine Vyers HCS/587 August 20, 2013 Organizational Change Plan -Part 1 Abstract Rapidly changing patient care environments result in significant ramifications for nursing team members caring for patients. These recent changes have refocused the current workflow views back onto inter-disciplinary teams, and collaborative working environments, and lead to a reexamination of nursing roles in the Long-Term Acute Care (LTAC) arena. Using action research methodology allows for the identification of areas of change, clarification of core and specialty skills, which in turn assist in the redefinition of nursing roles and eventually the adaption of a new nursing model. The new nursing model requires ongoing, detailed, and process driven education to establish and clarify the change objectives and new nursing roles. Quantitative and qualitative data collected from numerous sources, in conjunction with unit-based round tables, individual patients, nurse managers, and administrative interviews allows for consideration of multiple stakeholders. When patients vocalize a positive experience with services provided, this information helps to shape the new roles and responsibilities for the nursing teams. Alcoholism is considered a disease, meaning that it follows a characteristic course with known physical, psychological, and social systems. The alcoholic continues to consume alcohol despite the destructive consequences. Alcoholism is serious, progressive, and irreversible. If not treated, it can be fatal. It is generally thought that once the disease has developed, the alcoholic will not drink normally again. An alcoholic who abstains from drinking, however, can regain control over the aspects of life with which ALCOHOL interfered. The alcoholic is then said to be “recovering” not “cured” of the disease. It is important to note that the particular symptoms and pattern of DRINKING PROBLEMS may vary with the individual. ...Risk and Protective Factors for Depression: Implications for Prevention Depression in Children and Adolescents Linking Risk Research and Prevention Judy Garber, PhD Abstract: The National Institute of Mental Health has called for translational research linking basic knowledge about vulnerabilities that underlie mood disorders to the development of effective preventive interventions. This paper highlights research about risk factors for depression in children and adolescents and links it to current knowledge about interventions aimed at preventing depression in youth. Basic epidemiologic and clinical research indicates that increased risk for depression is associated with being female; a family history of depression, particularly in a parent; subclinical depressive symptoms; anxiety; stressful life events; neurobiological dysregulation; temperament/personality (e.g., neuroticism); negative cognitions; problems in self-regulation and coping; and interpersonal dysfunction. These vulnerabilities both increase individuals’ chances of encountering stress and decrease their ability to deal with the stress once it occurs. Although several existing depression-prevention studies have targeted one or more of these risk factors, the efficacy of these various prevention programs for youth with different combinations of these risk factors needs to be investigated further. Berlin, ScD; and Eileen E. Reynolds, MD Background: The risks and benefits of mammography screening among women 40 to 49 years of age remain an important issue for clinical practice. Purpose: To evaluate the evidence about the risks and benefits of mammography screening for women 40 to 49 years of age. Data Sources: English-language publications in MEDLINE (1966 – 2005), Pre-MEDLINE, and the Cochrane Central Register of Controlled Trials and references of selected studies through May 2005. Study Selection: Previous systematic reviews; randomized, controlled trials; and observational studies. Data Extraction: Two independent reviewers. Data Synthesis: In addition to publications from the original mammography trials, 117 studies were included in the review. Metaanalyses of randomized, controlled trials demonstrate a 7% to 23% reduction in breast cancer mortality rates with screening mammography in women 40 to 49 years of age. Screening mammography is associated with an increased risk for mastectomy but a decreased risk for adjuvant chemotherapy and hormone therapy. The risk for death due to breast cancer from the radiation exposure involved in mammography screening is small and is outweighed by a reduction in breast cancer mortality...
