1000 Head to toe assessment completed. Pt in room lying down in bed. PICC on left side without redness or swollen. A&O X4. Vital sign T 97.7, P 75, R 20, BP 108/70 and O2 96 RA. Pt stated pain 2 on the lower left leg on scale of 0 to 10. Pt looks discomfort. Hair clean and dry and evenly distributed. PERRLA. No drainage tenderness and redness on nose. Ear clean without redness and swelling, small amount of cerumen presented in ear canal. Oral mucosa pink and moist. Dentition intact no bleeding in gums or swollen. Non palpable or non-tender on lymph gland. Full range of motion and symmetrical. Capillary refill < 2 second.
