...Health Self Assessment In order to improve one’s health and quality of life, it is important to be aware of an individual’s health status. Our textbook authors, Meeks, Hait & Paige (2009) describe the importance of self-appraisals and health behavior inventories in teaching students about their practices that will impact their health. In order to become an effective teacher who is enthusiastic about health education, the author of this paper focuses on gaining a comprehensive understanding of her health. After assessing my health through an internet tool available on the Council for Responsible Nutrition wellness campaign webpage, I have reached several conclusions and understanding of my current general health status. The health assessment tool on the webpage is called “my wellness score card”. The assessment analyzes nutritional choices, use of supplements, habits, exercise, and healthy weight relative to gender, height and age. Prior to this assessment of my lifestyle, I was aware of some of my bad habits but I was under the impression that I did enough to balance it. Once I read the results, I was somewhat shocked! After careful thinking, I believe that finding out where I stand now, even though I suffer from few illnesses, can be beneficial, almost a wakeup call. Being aware of my habits and lifestyle choices is the first step in the right direction toward improving my health status. Below is the graph of my assessment results from the Council for Responsible Nutrition...

... Dear Mr. Alberto M. Carvalho: I have been concerned with what is happening to our children these last few years. Childhood obesity is becoming more prominent and should be on your priority list. I am currently a Registered Nurse enrolled in the University of Phoenix to obtain my Bachelor’s degree in Nursing. I have become more aware of my community and through different assessment in the assignments; I have had the chance to identify some of the different characteristics within my community that could adversely affect the health of the population. Childhood obesity is one of the factors that caught the most of my attention. My job now is to respectfully ask for conscientiousness and to educate the Board Members of precipitating factors that are contributing to this countywide epidemic. With rising costs and a slowing economy, I commend your efforts to explore additional ways the Dade County School Board can implement a more cost-effective margin with the various food programs you have recently implemented. As you know, there are always pros and cons to everything. During my community assessment, the main factor that was noticed was how obesity is presently harming and affecting our community’s health. ...Running Head: FAMLIY HEALTH ASSESSMENT 1 Family Health Assessment NRS- 429V Family Health Assessment 2 As a nurse, it is vital to remember that the family of the given patient plays a large role in their promotion of health. Family health practices, whether effective or ineffective, encompass activities performed by individuals or families as a whole to promote health and prevent disease (Edellman, 2004). The Gordon’s eleven Functional Health Patterns Assessment is a useful tool in assessing and evaluating the family’s developed health patterns, allowing the health care provider a better look at the overall health of a family as a whole. This allows for interventions to be uniquely crafted based on the findings of the assessment, ending with a result of better health outcomes. This paper will discuss the health habits and perceptions of the Foster family which was interviewed by this writer using Gordon’s assessment. Open ended questions were formulated to get an idea of how this family functions related to health promotion and the overall perception of their well being. The Foster family is a family consisting of a wife and husband in their fifties and a son who is 29. The wife and husband, M. Foster (wife), and D. Foster (husband) live alone, being that their child is grown and has moved out. The first assessment question is how the Foster family perceives their health. ...Personal Health Assessment and Health Promotion Plan Abstract The purpose of this assignment is to gain a greater understanding of the health promotion concepts through self reflection. I plan to do this by identifying three self-assessment tools; a food journal, exercise log and sleep log, along with providing rationale for the chosen instruments. I will also address the Transtheoretical Model (TM), which stages I have gone through and which stage I am currently in. These tools and stages along with a wellness diagnosis will help me to develop a personal health plan. The outcomes of the interventions will be discussed and will include personal achievements and success. Personal Health Assessment and Health Promotion Plan The definition of health is very subjective; it can vary from person to person. What one person might consider healthy another might consider being average. I define good health as a complete circle of one’s physical, emotional, spiritual and mental well-bring. Personal health assessment is a key building block in understanding your own health. Once there is an understanding one can make plans for improvement. According to Pender “a thorough assessment of health and health behaviors is the foundation for tailoring a health promotion-prevention plan” (Pender, Murdaugh, & Parsons, 2011, p. 89). Once my definition of health was clearly defined, I took a closer look at my personal health beliefs by completing the Health Beliefs Survey (Appendix A)...
