BEHAVIOR OBSERVATION: Mrs. Heather Sumrall was well dressed, Married 34, Caucasian female who was present for interview and testing in a timely manner. She presented herself and forthright manner with no indication of misrepresenting her history or psychological complaints. Mrs. Sumrall was alert and oriented. Her speech was clear, logical and goal directed. Her mood was bright with congruent affect. She seemed to have good awareness and motivation to make positive changes in her life for weight management.
PERSONAL HISTORY: Mrs. Heather Sumrall is the older of two children born to her biological parents. She was raised in a rural area at the middle-income level. Her parents separated when she was 6 to 7 years of age. Her father left her mother…show more content… Sumrall's performance on the Verbal Scale of the WASI places her at the 90th percentile and in the High Average Range of Intellect.
Her responses on the Overeating Questionnaire indicated she presented an accurate and consistent description of her eating patterns. She does not have a tendency to eat after hunger satisfied. However, she does tend to engage in unsafe dieting practices. Her cravings are moderate and she tends to eat moderately as a way to achieve emotional regulation. Mrs. Sumrall rationalizes her in behavior, but her motivation to maintain a regimented diet program is good. Her body image is not tied to her personal identity. She does tend to be self-conscious because of her bodyweight. There was mild sadness and anxiety…show more content… Heather Sumrall will have no difficulty interacting with staff and the areas of verbal and written materials. Although testing showed no marked clinical psychopathology, she is having sub clinical symptoms, which seems to be enough to warrant medication reevaluation. She apparently has a history of anxiety and a recent diagnosis of attention-deficit hyperactivity disorder suggested a diagnosis of bipolar II. She denied any significant trauma or drugs or alcohol related dependence. It is believed psychotherapy would be helpful for her and was mentioned. Mrs. Sumrall is aware of maladaptive eating behaviors and is willing to make the commitment and engage in long-term lifestyle changes to reach her goals. No suicidal ideation was noted or reported. She does appear willing to forgo foods as a means comfort source. Mrs. Sumrall does appear to have suitable coping mechanisms is in a current, stable marriage with good work history. She is willing to attend follow up in-group support meetings. It is believed ms. Sumrall would be a good candidate for bariatric surgery if she simultaneously seeks medication
