Personal Wellness Plan Personal Wellness Plan A person’s health is one of the major assets for him. During his whole life, he tries to preserve it in order to lead a healthy life. In my case, I have greatly abused my health state in recent time. When I was in India, I used follow a healthy diet plan thanks to my mother. But when I came to Canada, my diet plan degraded drastically. My health is downgrading which I feel many-a-times. I suffer from acute migraine pains, low hemoglobin and high stress. Sometimes I feel sudden blackouts when working under stress full situations. I have been frequent to clinics due to my poor health. These all problems not related to my family medical history. My father and mother are enjoying a healthy life due to their healthy food diet. My level of physical activities are fair and I try to keep it moderate with my daily calorie intake. My everyday diet roughly consists of following items: * Breakfast – Cup of Milk accompanied with couple of bananas or apples. * Lunch – Cup of regular coffee sometimes with a donut or a bagel. * Dinner – Any junk food which is available from any nearest outlet.
Compared to “Health Dimensions”, my health status in general is not good. Health Dimension | My pre-test rating | Physical health | 2 | Social health | 3 | Emotional health | 2 | Environmental health | 2 | Spiritual health | 2 | Mental health | 2 | Occupational health | 4 | My physical health is somewhat unhealthy. My condition of health is mostly due to my unhealthy diet and less physical activities. My social health is somewhat healthy. Except my occupational health, rest all of my other health dimensions carry a rating of 2. Only my occupational health have a rating of 4.
My Goal
My goal is to bring my health back to the track on which it was going on earlier.
