The decision making used definitely follows the garbage can model because things were done very sloppily. The minimal decision making that occurred ended up having very dangerous consequences. There was definitely not an orderly system of steps being followed and that ultimately probably lead to the unfortunate death of those workers. Representativeness heuristic was definitely present even though nothing was done about it.
2. In order to have used any type of evidence based decision making, the company would have had to do research. It seems like the company did not do much research on the safety concerns of the oil rig. It also seemed like there was a lot of confusion on whose job it was to complete certain tasks. They also should have worked on implementation procedures so that things were actually getting accomplished.
3. I think these issues were caused by a complete lack of planning. There also seemed like there was no way for anyone to know who was assigned to do which task and who was supposed to facilitate each task on the oil rig. There definitely should have been someone assigned to monitor the dangerous levels on the rig. The lack of coordination between the two companies definitely lead to the explosion of the rig. There also should have been no misunderstanding of the data because the data is very important in this case.
4. This case really goes to show that poor planning can lead to terrible outcomes. It is obvious from this case that if you do not plan out who is supposed to do what, things will not get done. It is important that someone is also assigned to monitor things that need to be checked frequently so that they know if levels of certain things are getting dangerous so that actions can be taken

With the current emphasis on high-stakes testing, educators are trying to do more with less, which can result in an overabundance of schoolwork outside of school. Some critics say there is no evidence to suggest that homework is helpful to student achievement; on the contrary, too much of it can overwhelm students and cause them to disengage. Others, however, claim that homework is necessary and helpful, designed so students can practice the concepts taught in class, build good study habits, and reflect on their own learning. As an eleventh grade student who is in advanced and ap classes, I think that homework is the cause of my depleting grades. This has also kept students busy every night, staring at their computer screens studying or researching about their assigned school work. Homework, in a way, is a good way of maintaing the knowledge students gained from school, but is it really the most effective way of providing supplemental studies? Most studies have shown that homework may be more supplemental than being detrimental to a student’s learning. On one perspective, it has been proven that assigning homework is beneficial to a student’s performance in school. Evidence has been found that homework is an effective supplemental form of learning, and “can impact positively on the retention and understanding of knowledge and can improve study skills...and demonstrate that learning can take place outside of formal schooling,” (qtd. in Department of Education and the Arts 7). This shows that studying should not just be confined to the walls of the classroom, and that after-school learning is vital to reinforce what has been taught by the teacher to the students.
