An ISP meeting was held on October 29, 2015 for Linda Oliver at Houston County Association for Exceptional Citizens in Warner Robins, Georgia. All in attendance and participated in the meeting was Linda Oliver (Individual), Betty Hugley (Group home manager), Jaquetta Moore (Supervisor), Wallace Pitts (Supervisor), Jennifer Dalton (Social service manager) and Lashonda Turner (Support Coordinator and Facilitator). Linda's legal guardian and sister, Cynthia Padgett, was unable to attend the meeting however permission was given to conduct the meeting in her absence.

Linda was in a good mood. She was upbeat as she verbally expressed herself.

The team discussed Linda's recent service change as she recently discontinued her prevocational services. …show more content…
Linda was also recentely referred to a specialist for the treatment of Parkinson diesease however she was not diagnosed with the diesease. During the followup appointment the physician recommended that Linda's thorazine medication be slowly reduced to discontinuation.

In addition to the results of Linda's medication, Linds is needing more assistance with walking as she is no longer confident in walking independently. The team discussed the issues Linda has with dept perception and the helps she refuse to utilize. SC and Linda's legal guardian discussed the need for glasses which could possibly aid her with her dept perception however both parties agreed Linda will not utilize …show more content…
Training considerations in the area of toileting is to Educate caregiver regarding behavioral assistance needed for the prevention of incontinence. This includes the use of a toileting schedule, incontinence products, appropriate hygiene practices and safety measures. Training considerations in the area of skin breakdown is to instruct the caregiver on the ways to minimize impact of behavioral issues on skin integrity. The other areas of training consideration service requirements can be found in Linda's HRST. No assessments have been recommended. Staff has been trained in all areas of need. Provider agreed they will monitor and give prompts as per HRST. The team feels that training concerns are being addressed and that there is no need for initial implementation of training within the next 30
