According to Nevitt and Cummings, many factors con¬tribute to determining whether a specific fall results in an intertrochanteric fracture:
a) A person must land on or near the hip in order for the energy of the fall to be transmitted to the proximal femur; falling onto the lateral thigh or buttock near the greater trochanter is much more likely to cause trochanteric fracture than affect elsewhere. Such falls are also much more likely when there is slight or no forward momentum, as when the person is standing still or walk¬ing slowly, which is another factor that helps explain why the elderly suffer a greater proportion of fractures in these inci¬dents. Furthermore, because their reaction times are longer and muscle strength less, older persons' protective responses tend to be minute and delayed.

There is hope that intertrochanteric fracture risk has begun to descend in certain areas of the world, but the cause is unknown. In Denmark the incidence of intertrochanteric fractures has declined about 20% from 1997 to 2006; nonetheless, this decline cannot be clarified by administration of anti osteoporotic medications, whose effect should only be an approximate reduction of 1.3% in men and 3.7% in women. The incidence of intertrochanteric fractures is highest worldwide in Scandinavia. In Finland, the number of hip fractures among older people almost doubled between the early eighties and early nineties, but since then, a decline in intertrochanteric hip fracture incidence has been observed both in Finland and around the world.
