Master File is a “Mission Critical Database” that stores the taxpaying histories of 227 million individuals and corporations, including every transaction between taxpayers and the IRS for the past 40 years. The Master File is used to determine if you've paid what you owe, and without it the government would have no way to flag returns for audits, pursue tax evaders or even know how much money is or should be flowing into its coffers. The system is poised for a “FATAL CRASH” that would “SHUT” the government down. It still runs code from 1962, written in an archaic programming language that almost no one alive understands. The intricacies involved in updating this age old database have made it a “Rube Goldberg Machine”. Modernization Project The main objective of the project is to implement centralized database called CADE (Customer Account Data Engine) to replace “Master File”. Apart from replacing “Master File” with “CADE”, the project is also aimed at modernizing 100+ business applications used by IRS. What once was a problem predominantly in the southern counties of Maine started to climb farther and farther north as the paper mills and the towns they supported rolled up the sidewalks and shut their doors for good. Suddenly, the mill workers, lobstermen, and construction workers were out of a job. The loss of employment led to the loss of affordable insurance. The lack of insurance made previously affordable medications more expensive, sometimes by quite a margin. The loss of programs and activities for youth due to hard economic times contributed to the rise of drug abuse among the younger, school-aged generation. Additionally, easier access to diverted prescription opioids was at an all time high. The presence of these factors in tandem produced a generation with an unprecedented addiction rate. Traffic: The Danger Comes North After the better part of two decades, freely-prescribed opioids were drying up everywhere. The medical community were wising up to the mistakes they had made. As the demand for opioids among a deprived population grew, however, it opened the door for other opportunities within the drug trade, like the movement of heroin and its astronomically more dangerous counterpart, fentanyl. This is in contrast to much of the world, where the 1978 Declaration of Alma-At a which recognized that attaining health for all also requires interaction from social and economic sectors - is considered standard. Today, there is much buzz about patient-centered medical homes, a concept that promises to transform the practice of American medicine. There is much to praise about this most recent iteration of the medical home. But the missing ingrethent in all these definitions and models remains public health. A population focus that addresses the social determinants of health is an essential component of primary health care. In the United States, such a comprehensive approach has been labeled community-oriented primary care. This model is built firmly on the Alma-Ata principles and incorporates a public health approach to health services. Community-oriented primary care organizes the delivery of health services, around a population, not simply a collection of individuals. It identifies a population - most frequently a geographically defined community - and uses epidemiology and interventions to improve community and individual health and well-being. In this model, both individual patients and the community are the foci of the delivery of health services. Primary health care stands at the intersection of personal and population health services. At the end of the day, Jennifer and George were heading back to their vehicle when Jennifer noticed a go-kart track. The two entered the track, bought tickets, and began riding. Within one minute, Jennifer’s go-kart bumped into one of the side guardrails, flipped on its side, and burst into flames. Jennifer was seat-belted in the go-kart and could not get out. George tried to run into the fire to save his sister, but the flames were too intense. Bystanders attempted to use a fire extinguisher, but it did little to lessen the inferno. Jennifer was trapped in the burning go-kart for two minutes when her seat-belt finally burned through and she fell to the ground. George grabbed his sister and pulled her away from the fire. Jennifer was alive. She was lying on the asphalt alert, oriented, and coherent. She had suffered 3rd and 4th degree burns covering ninety-five percent of her body. She was suffering the worst pain imaginable. Within the past few years, numerous examples of data such as Social Security, bank account, credit card, and driver’s license numbers, as well as medical and student records have been compromised. A major reason for the increased awareness of these security breaches is a California law that requires notice of security breaches to the affected individuals. This law, implemented in July 2003, was the first of its kind in the nation. State data security breach notification laws require companies and other entities that have lost data to notify affected consumers. As of January 2007, 35 states have enacted legislation requiring companies or state agencies to disclose security breaches involving personal information. Congress is considering legislation to address personal data security breaches, following a series of high-profile data security breaches at major financial services firms, data brokers (including ChoicePoint and LexisNexis), and universities. In the past three years, multiple measures have been introduced, but to date, none have been enacted. This report will be updated regularly.
