Canada Post ID Verification Form To have your ID verified by Canada Post, follow these simple steps: 1) Please print the “Canada Post ID Verification Form” and visit www.backcheck.net/canadapost to find a participating Canada Post Retail Location in your area. 2) Take the form, along with two (2) pieces of appropriate ID, and present them to a Canada Post retail clerk to verify your information. (Please read below for acceptable types of ID as determined by the RCMP.) 3) That’s it! The Canada Post retail clerk will scan the completed form and ID’s and send them directly to us for processing. Instructions for the Canada Post Retail Clerk 1. Obtain two (2) pieces of identification from the applicant. myBackCheck.com will provide acceptable forms of ID to the applicant via their website. 2. Compare the information in the Signature Block section below with the identification provided by the applicant and the likeness of the person appearing before you. 3. 4.
