...of losing her children. Assessment The assessment first begins with the usage of the Person-In-Environment Classification System. Under Factor I, the client’s ability to perform the parental role has been identified as a problem, specifically classified under the loss type. Her separation from her children, coupled with the impending custodial battle, has caused much anxiety and depression over the past few months. Under Factor ll, the economic and basic needs system and the judicial and legal system have been identified as areas of concern (Wandrei & Karls, 1994). In the economic and basic needs system, the lack of proper shelter and employment will be examined. During the time of the session, the client was living with her cousin and her young child in a one room rental flat in Ang Mo Kio. The ownership of the flat was said to be threatened as the loss of the custody over her children can cause her to become ineligible to rent a flat from the Housing and Development Board. This uncertain situation is thus identified as a problem. The client is also technically unemployed, although her previous employment has not been terminated officially. Previously, a bad gastric attack during work prevented her from performing her job and she was accused of slacking off, thus leading to her unofficial termination. The client has expressed desire to seek alternative employments, although there has been no results or more information gathered here. The judicial and legal system has also been...

...Analysis of CK and the K Family Jane Doe Introduction Family nursing is a fairly new concept relative to the history of nursing practice. With any type of nursing process, assessment is the first (and ongoing) part of the process, followed by care planning and intervention. In delivering comprehensive care, the nurse must incorporate assessment of the family unit, or lack there-of, associated with the identified patient. Many family assessment tools have been devised to assist the nurse in gathering accurate data while opening up lines of communication and reflection for all members involved in the process. Selection of the appropriate tool for the identified family is crucial. Effective tools must be easy to administer in a fairly short amount of time and the data collected needs to be immediately available and useable. Family members need to see the relevance of the information gathered, and how it assists in improvement of their care- ideally soon after discussion, either by the nurse or a clinician. An initial interview and follow up discussion with the K family was completed over the course of 3 sessions. The first encounter involved only the identified patient (IP). The second was the remainder of the family, and a third was with CK’s perceived family. After an initial assessment and evaluation of the K family a suggested direction of care emerged, but was re-evaluated after the second interview. This development was then paired with...

...solid foundation required to develop a plan and provide optimal care to their patients. This assessment is significant not only to individual patients, but their families, who are becoming increasingly recognized for their significance to the health and well being of individual family members. Nurses use a variety of tools in family nursing, and one of the most significant includes the Calgary Family Assessment Model (CFAM), developed by Wright and Leahey. CFAM is an integrated conceptual framework used for interviewing and making assessments of families. It consists of three major categories, structural, functional and development, and each category contains its own subcategories. This paper will demonstrate the assessment of my family, the Fournier Family, using the Calgary Family Assessment Model. Strengths and weaknesses of the family will be determined, information which is necessary for the development of three significant nursing diagnosis applicable to the family. Interventions to meet the outcomes of these diagnoses arise through collaboration between the nurse and family, as they develop strategies to promote, improve, and sustain effective family functioning. Through the use of this model, assessment, diagnosis and interventions will each be identified and applied to the Fournier Family in an effort to improve the families’ outcome as a unit. Central to the delivery of safe and effective family nursing care is the nurse’s ability to make accurate assessments, which...
