Mr. Correia is 26 year old male who presented to the ED with an BAC of .23 and overdose on 12-15 Benadryl and Mortin PM. Per documentation Mr. Corriea reports to EMS that he wanted to die. Mr. Correia denies suicidal ideations, homicidal ideations, symptoms of psychosis. He does not appear to be responding to any internal stimuli.

During the time of the assessment Mr. Correia was 4X oriented, appropriate speech, appropriate appearance, fair eye contact, and guarded. Mr. Correia reports a history of self harm by cutting when he was 15 years of age after his relationship ended. Mr. Correia reports last night he had consumed 30 or more beers and a few shot of liquor. He reports taking 4-5 sleep aids a night to help him fall asleep, however
