Abstract

Nutritional rickets remains a public health problem in many countries, but mostly problematic in developing countries where they lack vitamin D and adequate ultra-violent light exposure. This disease is seen in younger children ages six to twenty-four months, especially in the infants that are primarily breast-fed. This is because breast milk is low in vitamin D. The disease can also appear in toddlers and older children it is said that this is due to low dietary calcium intake. This can be solved simply with calcium supplements.

Nutritional Disorders Rickets

Nutritional rickets is a disease that causes the bones to soften and often times break (7). When people think about it they normally visualize kids that have misshaped legs and arms and mostly kids that are malnourished. The causes of this disease are a vitamin D deficiency, calcium deficiency and the lack of ultraviolent light exposure (1). Despite the medical advancements nutritional rickets remains a major problem in developing countries. In this paper, I will review what the disease is, the causes, how it is diagnosed, signs and symptoms, how to treat, and the prognosis.

Nutritional rickets also known as osteomalacia in adults causes the bones to soften and to break (2). This disease is mostly seen in children ages six to twenty-four months. It is more prevalent to kids in this age range due to them solely being breast-fed and there is a lack of vitamin D in breast milk(5). Nutritional rickets is also seen in toddlers and older children and this is primarily because of the lack of calcium intake. This disease is primarily seen most in developing countries due to the lack of availability of vitamin D, calcium and ultraviolent light(3).

Nutritional rickets is primarily caused by lack of vitamin D. With the lack of vitamin D, calcium is not absorbed properly this causes hypocalcaemia (lower than normal blood calcium)(7). When this happens the body starts to pull calcium from the bones in order to get the levels back high in the blood stream. The end result of that is nutritional rickets. In some areas the lack of sunlight is also an issue. Not only is vitamin D taken in to the body, it is also produced by the body. The skin cells react with ultraviolent lights (sun light) to help create vitamin D.

Nutritional rickets can be diagnosed with a blood test, to see the levels of calcium in the blood. Doctors can also do an x-ray to look at the growth and shape of developing bones. Some doctors even do a physical exam and these exams show a tenderness or pain in the bones instead of in the muscles and joints. Some even do what’s called a bone biopsy. A bone biopsy is when a sample of the bone is removed and viewed under a micro scope to look for infectious diseases or cancer. This procedure is rarely done to diagnose nutritional rickets.

There are several sign and symptoms of nutritional rickets. The most obvious sign is bone deformities, for example knocked knees(8). This occurs when kids try to stand and their knees and legs bend under the pressure. Teeth growth will be delayed and when the teeth do come in they will be soft and more prone to cavities. Another sign is Costochondral swelling, which is when large beads show up under the skin of the rib cage (11). Also there is what’s called Harrizon’s groove, which is a visible horizontal line where the diaphragm attaches to the ribs (6). A couple of other signs are growth delay in height and weight and uncontrolled muscle spasms, which may affect the entire body.

The goal of treatment is to correct the cause of the condition. In most cases it is to simply give the child more vitamin D and calcium. Doing this will eliminate most symptoms. In other cases kids will need to have their bones positioned and will have to where braces to correct the problem. While in some cases of the bone deformities kids will need corrective surgery. Also it is recommended to expose the kids to prominent amounts of sunlight.

If nutritional rickets is not corrected while the child is still growing the bone deformities and growth problems will be permanent. If it is corrected most deformities will disappear or they will correct themselves. Most patients start to show improvement within a week of treatment. Most patients live a normal and healthy life so the prognosis is good(9).

Nutritional Rickets is a disease that causes the bones to become soft and weaken which can lead to bone deformities and they can also break. This disease is mostly seen in younger children who are primarily breast-fed. The main cause of this disease is the lack of vitamin D. This disease is so prevalent in developing countries due to the lack of resources containing vitamin D. The prognosis of this disease is good if treatment comes into play during a child’s growth period.
