The applicant is a 42 year old male with four children aged 24, 13, 12, and 8. The applicant is divorced and co-parent with his ex-wife. He reports his ex-wife supports his recovery. The patient current living arrrangment is that the applicant lives at home with his parents.

The applicant reports he first started using drugs at the age of 24. The applicant reports to using cocaine and heroin, by IV. His last use of cocaine was about a month or two ago, a $20 bag. As for the heroin, the applicant reports to using it daily, by IV, 2 bundles. The applicants then reports he had used alcohol at the age of 16 and drinks occassionally. The applicant is aware about the effects of mixing alcohol and methadone and stated, " I haven't used any alcohol while receiving methadone treatment in the past, I know the risk. I will stop. I, was only drinking 1-2 beers once a month during social events and I was not in any program, but again, I will stop." The applicant reports smoking one pack of cigarrettes per day and wants to quit smoking. The applicant denies using any benzos, usage of designer drugs, street methadone, and street suboxone. …show more content…

The applicant reports he was enrolled at the ADRC detox treatment facility seven years ago and completed 5 days to address his heroin addiction. The applicant then reports to enrolling at a methadone clinic at the Hartford Dispensary. Again, the applicant struggle with remember the year and how long he was in treatment. He would say, " It was 2015....no, it was 2014" then says, " I was there for 8 months, wait...actually, I think it was a year or more." The applicant reports he believe it was 03/2014 and was discharged because of his continously use of heroin from the Hartford Dispensary. The patient signed an ROI for ADRC and Hartford

...My road to learning of how to read and hearing any rhymes was not the same as the usual child born here. My parents are of Hispanic heritage, and they spoke to me in Spanish mainly making that be my first language. I was homeschooled for my pre-school year, so I had someone come and teach me how to read and learn songs in Spanish. This way was not as difficult for me to grasp for I had already knew some words on how to say, since my parents already told me those words. My very first memory of the book that I first read was on “If you give a Mouse a Cookie.” This book was taught to me in Spanish, but I remember it clearly for teacher at the time brought a stuffed mouse and cookies to help me understand the story better. My teacher made me have physical examples of the story making me not feel it is a difficult book to read. However, once I entered a public school, they immediately made me learn English right away which was a challenge for me since it was a small child learning a whole new language all over again.
