Nurses are urged to integrate and use evident based practice in everyday nursing. Evidence-based practice (EBP) is a problem-solving approach to the delivery of health care that integrates the best evidence from studies and patient care data with clinician expertise and patient preferences and values (Melnyk et al, 2009). I have been working at Virginia Beach psychiatric center and during a replacement on psychiatric rapid stabilization unit this nurse noticed that medication nurses face many disruptions while conducting court order medication rounds and witnessing nurse considered ways to minimize disruptions and thus develop nurse and patient safety. Disruptions are unavoidable because everybody must respond to broadcasted code when alerted. The author came up with a research question using the PICOT acronym, ‘do disruptions during court order medication rounds increase the medications errors made by nurses. The research question in this situation is to address the patient and nurse safety and using the PICOT structure to address the issue of disruptions during court order medication round (as perceived by this nurse), the ultimate research question was articulated: ‘do disruptions during court order medication rounds increase the medications administration errors made by nurses?
PICOT is an acronym for the elements of the clinical question (Melnyk et al, 2009):
P -Patient or Population/Problem
I -Intervention
C -Comparison
O -Outcome
T -Time

Intervention PICOT Question P (patient population) | Nurses on court order medications rounds. | I (intervention or issue) | How do disruptions during court order medications rounds. | C (comparison) | no comparison in this case | O (outcome) | A reduction in medication administration errors | T (timeframe) | During 1-2 weeks hospital stay |

...Running head : UNDERSTANDING ENDOMETRIOSIS 1 UNDERSTANDING ENDOMETRIOSIS 2 Endometriosis is a condition when the lining of the uterus grows in other places. The lining of the uterus sheds and that’s what creates the menstruation cycle. The lining grows in the fallopian tubes, along the pelvis and the ovaries that cause heavy flow and sever cramps. Over 6 million women in America suffer from this condition. Endometrial implant occurs primarily in the abdominal and pelvic area of the body. The endometrial breaks down and bleed in conjunction with a female normal menstruation cycle. This break down of the lining causes inflammation and pain. The inflammation can lead to further complications like fibrosis, scarring and adhesions. (Huether, S & McCane, K, 2012) The cause of endometriosis is unknown. Some say is caused by the backflow of menstruation. They also say it can be a genetic disorder. Know one knows the exact cause of endometriosis but it can have some serious effects. The symptoms of endometriosis are pelvic pain, dypaurnia, dyschezia and abnormal vagina bleeding. (Huether, S & McCane, K , 2012). Some people...

...PICOT Question PICOT Question As a Hispanic American, the realization that my family and I are at increased risk of receiving a diagnosis of Type 2 Diabetes is a real consideration. Within the last 2 years both my mother and father have been diagnosed with this disease. Additionally, all of my grandparents have been diagnosed with Type 2 Diabetes and two have died as a result of comorbidities associated with this disease. It is estimated that Hispanics are at 50% to 100% increased risk of experiencing burden of illness and mortality compared to non-Hispanics (Two Feathers et al., 2005, p. 1552). Additionally, the elimination of racial and ethnic disparities regarding type 2 diabetes risk factors have been identified as a goal for the Centers for Disease Control and Prevention (Two Feathers et al., 2005, p. 1552). The discipline of nursing is one that is concerned with human responses and in affecting improved health and wellbeing in the communities we serve. Therefore, it is in the realm of nursing to understand and remove barriers to improve health outcomes in Hispanic Americans with newly diagnosed type 2 diabetes. Therefore I pose the following PICOT question: How do adult Hispanics with newly diagnosed type 2 diabetes perceive barriers to implementing dietary and lifestyles changes to reduce A1C levels in the first 3 months after diagnosis? P- (population) adult Hispanics with newly diagnosed type 2 diabetes. I- (issue of interest) perceived barriers...

...The PICOT question format is a consistent "formula" for developing answerable, researchable questions. Writing these questions is deceptively simple! When you write a good one though it makes the rest of the process of finding and evaluating evidence much more straightforward. As strange as it sounds deciding what you are looking for related to your outcome can be one of the hardest parts! When you determine the outcome it helps you hone the process of searching for evidence. Remember evidence is not just what you want the outcome to be... it is evaluating the evidence supporting or refuting what influences the outcome. If you are interested in getting to the heart of the effect of testing Foley balloons on the rates of UTIs then you need to find evidence on the pros and cons of testing the balloon! Question Templates for Asking PICOT Questions: P: Population/disease (i.e. age, gender, ethnicity, with a certain disorder) I: Intervention or Variable of Interest (exposure to a disease, risk behavior, prognostic factor) C: Comparison: (could be a placebo or "business as usual" as in no disease, absence of risk factor, Prognostic factor B) O: Outcome: (risk of disease, accuracy of a diagnosis, rate of occurrence of adverse outcome) T: Time: The time it takes to demonstrate an outcome (e.g. the time it takes for the intervention to achieve an outcome or how long participants are observed). Note: Not every question will have an intervention (as in a meaning question)...

New Picot Question

...Picot exemples Around five years back, our hospital intensive care unit switched to using wipes for daily bathing and cleaning patients. I assume this was based on studies that wash clothes and basins can be a source of infection. Two kinds of wipes were introduced at that time. One was 2% chlorhexidine gluconate (CHG) wipes and the other was regular wipes (we called them peri wipes) which we used for the face and perineum. The general impression was that the CHG wipes could not be used for the perineum as it was too harsh for the sensitive skin in the face and perineum. Even though we learned later that the CHG wipes are safe to use in the perineum region (as per the Manufacturer), old habits die hard and we continue to use the peri wipes for cleaning the perineum in both urinary catheterized and non-catheterized patients. This is absolutely okay for non-catheterized patients but since then I always wondered if using the CHG wipes in the perineum during daily baths and routine periuretheral cleansing in catheterized patients would help in bringing down the rates of catheter associated urinary tract infection. If the evidence is strong enough, this practice can be implemented without any additional cost to the organization as we already use CHG wipes for the rest of the body. Also, there would not be any additional time or effort to be invested by the direct-care nurses as daily baths and routine periuretheral cleansing is already the current practice. Based on this...

...Frost Disseminating Evidence Dianna Frost Grand Canyon University Professional Development NRS-441V November 04, 2012 Frost Disseminating Evidence Disseminating the evidence from this program to community health nurses (CHM) is vital. The CHMs in this ASU nurse-managed health clinic (NMHC) in downtown Phoenix are in particular advantageous position to help address the problem of maternal obesity, because their primary cliental is college age students as well as other women of childbearing age in the local community. This evidence supporting preconception care will be presented to the clinical board for the ASU-NMHC, as well as financial supporters for the unique position they are in to drive this cost effective program to improve the health of future generations. These CHM encounter women of childbearing age during clinical visits and can begin to provide preconception education during clinical visits. They can be proactive in identifying those whom are at high risk for adverse pregnancy outcomes. Education about the effects of high risk health behaviors early before pregnancy and the effect on future children will reduce adverse pregnancy outcomes. Women who are appropriately informed with evidenced-based education will be empowered to make better choices about their health. They will be more likely to adapt positive health strategies to reduce a BMI greater than 30 with improved nutrition and physical activity, as well as decrease high risk behaviors. CHMs can also...

Picot Literature Review

...PICOT Literature Review Mia Fisher Grand Canyon University EBP 325 Wendy Cockron PICOT Literature Review This literature review examines the available research on the topic of the administration of natural surfactant vs synthetic surfactant in neonates with respiratory distress syndrome. Respiratory distress syndrome is caused by a deficiency, dysfunction, or inactivation of pulmonary surfactant and is a significant cause of morbidity or mortality of preterm infants. The literature reviewed, highlights the differences in extracted natural surfactants and synthetic surfactants and whether one proves to be more beneficial over the other. Pfister RH, Soll R, Wiswell TE. Protein-containing synthetic surfactant versus protein-free synthetic surfactant for the prevention and treatment of respiratory distress syndrome. Cochrane Database of Systematic Reviews, Issue 4. Art. No.: CD006180. DOI: 10.1002/14651858.CD006180.pub2. In the above article the use of synthetic surfactant is discussed with regards to protein containing or non-protein containing synthetics. The synthetics that contain proteins have shown a higher efficacy in preventing respiratory distress syndrome than those without. The research suggests the reasoning for this is due to the synthetic containing proteins acting like naturally occurring surfactant proteins. This research lends to the rationale that the synthetic containing protein surfactants effectiveness being like that of a natural surfactant...

...PICOT is an acronym for the elements of the clinical question: patient population (P), intervention or issue of interest (I), comparison intervention or issue of interest (C), outcome(s) of interest (O), and time it takes for the intervention to achieve the outcome(s) (T). When Rebecca asks why the PICOT question is so important, Carlos explains that it's a consistent, systematic way to identify the components of a clinical issue. Using the PICOT format to structure the clinical question helps to clarify these components, which will guide the search for the evidence. A well-built PICOT question increases the likelihood that the best evidence to inform practice will be found quickly and efficiently. Reflective Essay On Analysis Methods

...towards obtaining a DNP degree at Chamberlain College of Nursing, I will proudly say that I have completed a sixteen week of my DNP journey that the ANCN has advocated for. Hence, the following account is a self-reflection of my experience in the application of analytical methods course. Over that past eight weeks of the application of analytical methods course, we dealt with different topics, assignments, and discussions that range from PICOT question development to DNP project dissemination. I learnt the importance of evidence-based practices (EBPs) and their relevance to my current job as a nurse practitioner, and most importantly to my upcoming DNP project. My learning experiences during this eight weeks course section has been quite versed. In the first week, the class objective was to formulate a PICOT that will be used for an EBP intervention. I vividly could remember how I struggled to put a one-sentence PICOT question for high blood pressure education for African American adults. Later, I was able to come up with a better PICOT question at the end of the sixth week of class with the help of the class instructor. Similarly, I learnt about EBP models for practice. Examples of such models are: the ACE star (2004); IOWA (1994); Stetler and Marram (1976); Rosswurm and Larrabee models (1990); Alice Magaw (1902)….. Although these models have been in existence for quite a bit, I found them relatively new. In the class discussions, we discussed at length on how EBs and models...

The Gaza Strip

...Palestine has been recognized as a common ground for the constant and continuous conflict between the Jewish and Arabic communities. Through these situations, outer forces, such as Britain, have attempted avaricious approaches to the predicament, which ultimately led to the eruption between the two parties[1]. Beginning with World War 1, The Sykes-Picot Agreement, and the Arabs, following through to The Balfour Declaration with the Jews, showed that the respective parties were forced to rely on outside forces to help mend their broken hearts full of nationalism and possession[2]. It is safe to say that these two breeds of strong-hearted individuals made an impact in more places than Palestine itself. With Britain’s reign over the land, it was the duty of the Arabs and the Jews to get along with one another as best as they could, provided that Britain would give each party what they were owed[3]. Britain and The Balfour Declaration contradicted the original deal during World War 1 between Britain and the Arab community, sparking the conflict between the Arabs and the Jews, which, to this day, made living in Palestine a hostile and challenging place to call ‘home’. The Balfour Declaration, created in 1917, revealed the British offering Israel as a homeland for the Jewish people[4]. For the Jews, this meant that they would finally be able to have what is rightfully their own. Israel is the birthplace of the Jewish community, and living on the land where their story began has been...
