Class, Safian identifies the five essentials of documentation as Completeness, Accuracy, Timeliness, Accessibility, and Confirmation (Safian, 2009). These essentials overlap somewhat with the Data Quality Management Model provided by the American Health Information Management Association (AHIMA). The AHIMA lists eight governing principles of health care, including Accountability, Transparency, Integrity, Protection, Compliance, Availability, Retention, and Disposition (AHIMA, 2015). Although, the steps differ slightly, their aims are the same; to create air-tight documentation to protect both the health care organization and the patient.
