Six Dimensions of Health Worksheet

Part 1

For each of the following six dimensions of health, list at least one characteristic, activity, belief, or attitude that reflects that dimension in your life. Provide a brief explanation with each example. Refer to Ch. 1 in Health for explanations of these dimensions.

Physical health: I take my dogs for a walk every night for exercise.

Social health: I avoid isolating myself by calling or visiting with a friend or family member every day.

Intellectual health: I read at least one chapter of the book I choose once a day to stimulate my mind, and stay mentally active.

Environmental health: I keep my house clean and organized so I am environmentally healthy, and it also makes me feel happier.

Emotional health: I talk about my issues that are concerning me with someone before they build up inside and cause stress in my life.

Spiritual health: I read the bible and stay connected with my higher power at all times.

Part 2

In approximately 125 to 200 words, describe health and wellness in your own words using the ideas and concepts for each of the six dimensions of health.

To me health and wellness is a variety of different aspects in my life. These things combined together make up my health and wellness. Having good physical health means to me just simply doing what I can to try in stay active. This might not be enough but it is what I feel I am able to maintain on a daily basis. As well as being connected to other people provides social structure in my life, and helps prevent feelings of loneliness, and isolation. Reading is also another aspect of my daily attempts to stay healthy. I chose a book that interests me, and read at least one chapter every night before bed. Keeping my home and life clean and organized allows me to prevent illness
