Weber stated he was plowing the Steese Hwy, headed east, and when he came across the bridge he slowed down. He crossed the bridge and about ¾ of the way across, the plow dug in on the bridge deck and he ended up against the guardrail, with the plow above the guardrail. He stated he was going at about 15 mph when crossing the bridge. Furthermore, he stated that his supervisor’s instruction was to slow down when crossing bridges. And, to his understanding, he was to slow down to 15 mph because when his supervisor trained him, the supervisor was plowing at a speed of 30 mph and slowed down to 15 mph when crossing bridges; and that’s the way Mr. Weber has done it the past year. He stated that the supervisor wasn’t specific on the speed when crossing bridges; and the instruction to slow down to 1-2 mph was given to him the day after the accident. Mr. Weber stated that there hasn’t been any safety meeting regarding slow plowing. Weber didn’t follow direct instructions from his supervisor during his shift on October 16th when he decided to use the front plow, even though he was directly instructed to use the loader. In this instance, he failed to follow direct instructions from his supervisor because he didn’t feel the need to follow instructions.

Regarding the October 18th incident, management explained that Mr. Weber approached the bridge too fast, entered at an angle, he had crossed ¾ of the way across the bridge before the plow dug in, debris was thrown at about 100 feet, and the bridge was in good condition.
