DOB: 08/18/1958, Owner and Clinic Director of Stanton Chiropractic Clinic known as 7731 Katella Ave, Suite C, Stanton, CA 90680 and LINDA NISHIKAWA, D.C. DOB: 07/08/1969, Clinic Treating Doctor, and LAMSON WILLIAM NGUYEN DOB: 03/04/1967, Clinic Manager and Medical Billing.

ALL THE PARTIES AGREE AS FOLLOWS:

Subject to the term and conditions of this Agreement, all the parties will do the followings:

Clinic working hours will be Monday to Friday:
Morning: 9:00AM - 1:00PM and Afternoon: 2:00PM- 6:00PM and Saturday: 9:00AM – 1:00PM.
