Orchid View

...Orchid View Serious Case Review June 2014 Background In October 2013, an inquest was launched into the deaths of nineteen residents at the Orchid View care home in West Sussex, run by Southern Cross Healthcare. The inquest found that neglect had contributed to five of the deaths and that the other deaths had been caused by 'sub-optimal' care. The Coroner, Ms Schofield, commented on the appalling conditions at the care home stating that it was awash with 'institutionalised abuse'. In 2010 the Care Quality Commission (CQC) had rated Orchid View as 'good', Ms Schofield was concerned that such conditions and poor treatment of residents could go unnoticed by the authorities. In response to this, in June 2014, the West Sussex Adults Safeguarding Board commissioned a Serious Case review into the failings at Orchid View.   Findings The key findings of the Serious Case Review found lack of respect, dignity in relation to the treatment of service users. Maltreatment also included poor nutrition, poor hydration, left in soiled bedsheets, and mismanagement of medication. Call bells went unanswered and some were out of a service user reach. These fallings caused serious neglect that ultimately cost the lives of some service users. Recommendations 34 recommendations were made, for example: * Care companies should be required to provide evidence to the Care Quality Commission (CQC) that they can both recruit and sustain a skilled workforce. * Relatives to have a named contact...

...For the reasons which have determined the existence of Sussex as a county of England, and which have given it the exact boundaries that it now possesses, we must go back to the remote geological history of the secondary ages. Its limits and its very existence as a separate shire were predetermined for it by the shape and consistence of the mud or sand which gathered at the bottom of the great Wealden lake, or filled up the hollows of the old inland cretaceous sea. Paradoxical as it sounds to say so, the Celtic kingdom of the Regni, the South Saxon principality of AElle the Bretwalda, the modern English county of Sussex, have all had their destinies moulded by the geological conformation of the rock upon which they repose. Where human annals see only the handicraft and interaction of human beings--Euskarian and Aryan, Celt and Roman, Englishman and Norman--a closer scrutiny of history may perhaps see the working of still deeper elements--chalk and clay, volcanic upheaval and glacial denudation, barren upland and forest-clad plain. The value and importance of these underlying facts in the comprehension of history has, I believe, been very generally overlooked; and I propose accordingly here to take the single county of Sussex in detail, in order to show that when the geological and geographical factors of the problem are given, all the rest follows as a matter of course. By such detailed treatment alone can one hope to establish the truth of the general principle that human history...

For all this in order to be done, two West Sussex Councils - Adur District Council and Worthing Borough Council were hired as for assisting with all this. They were expected to give their best shot and to bring all the recent miseries to an end and to properly root them out. They were to come up with two operating councils, both in a working and goal oriented form. For this objective both were meant to work on their own, both facing their own issues and struggling to come on the same page. Their main purpose was to get all of things under monitored and to notice every little bit change in things regarding polices, fraud, transaction or any other thing. In today’s society, health care is a luxury indigent population cannot afford. Many families are made to choose shelter over health care when government assistance is not an option. Philosophy of Quality Management

...Abstract Patients are our primary customers. It is therefore important to measure their health care expectations and strive to meet those expectations 100% of the time. When a patient’s expectations are not reasonable, it becomes imperative to educate so that, over time, reasonable expectations are achieved. Often, perceived medical outcomes are poorly defined and arise from experiences from family and friends. Therefore, an opportunity exists to set the expectation of medical care and subsequent outcomes through education. Patient satisfaction has shown to relate directly to how well the health care team has informed the patient and family of the disease process and its treatment and whether they were allowed to participate in treatment decisions. For quality improvement strategies, health care professionals strive constantly in improving and developing the standards of care. They meet the challenges required of health care providers to be effective leaders who foster a culture and develop partnerships that embrace innovation. Quality delivery As consumer expectations rise, patient care becomes more complex, while resources shrink, leaving hospitals to find extraordinary means to define, organize, and staff quality assurance functions. Improving quality requires a unified hospital consensus about what quality means, who is responsible for it, and how to communicate those quality issues across the institution. Adaptation of a quality management method to organize...
