A prática de todos os enfermeiros estarem envolvidos inicialmente em ver todos os pacientes causou a duplicação de esforços, incluindo perguntas e exames repetidos, e resultou em gargalos processuais. Além disso, havia níveis inconsistentes de serviço e extrema variação em tratamento por causa da experiência e habilidade diferentes níveis dos enfermeiros. No geral, o ponto de vista dos pacientes era de que a clínica era ineficiente e impessoal. O sistema de triagem foi apresentado em Setembro de 1979 para superar estes problemas. Uma das mudanças imediatas foi estabelecer um sistema de avaliação e encaminhamento preliminar que dedica dois coordenadores de triagem, que são altamente experientes enfermeiros, para avaliar os pacientes e atribuí-los para a próxima etapa adequada no processo de tratamento.
