There is always opportunity for improvement of patient services within a health care setting. Total quality management (TQM) and continuous quality management (CQI) are processes used to improve services offered to consumers by placing the consumer first (Hood, 2014). The success of this approach relies on the premise that if staff involved in service delivery are empowered and educated on the process of incremental change, the quality and efficiency of patient care will improve (Hood, 2014). Although falls that occur are often harmless to the patient, the consequences can be serious and even result in death. It is the responsibility of each directly related staff member to prevent potential patient falls from occurring. When forming a team of members who are involved in this work process within my unit in a hospital setting, I would choose: doctors, nurses, certified nursing assistants, physical therapists, case managers, and dietary staff. In each patient room, an erasable sheet would be added on the wall next to each patient’s marker board. This sheet would list the hours of the day along the left hand column and along the top column would list the participating team members: nursing, nursing assistants, doctors, case management, physical therapy, and dietary. There is already an erasable marker on the ledge of patient marker boards in each patient room. When a member of the team interacts with the patient and then asks if they need to use the bathroom, they will check off the box for which team member asked and what hour they asked the patient. If the treatment team member is not a nurse or nursing assistant who would toilet the patient directly, they would then press the call light button and exit the room to find an available person to do so.
