1. The description the requirement of the video system is as below.

A video store system runs a series of fairly standard video stores. Before a video can be put on the shelf, it must be cataloged and entered into the video database. Every customer must have a valid customer card in order to rent a video. A customer rents the video for three days at a time. Every time a customer rents a video, the system must ensure that they do not have any overdue videos. If so, the overdue videos must be returned and an overdue fee must be paid before the customer can rent more videos. Likewise, if the customer has returned overdue videos, but has not paid the overdue fee, the fee must be paid before a new video can be rented. Every morning, the store manager prints a report that lists overdue videos. If a video is two or more days overdue, the manager calls the customer to remind him/her to return the video. If a video is returned in a damaged condition, the manager removes it from the video database and may sometimes charge the customer. a. Draw a use case model for the video system described above.

2. The case study below is about the patient treatment process in a clinic.

Whenever new patients are seen for the first time, they complete a patient information form that asks for their name, address, phone number and insurance carrier, which are stored in the patient information file. Patients can be signed up with only one carrier, but they must be signed up to be seen by the doctor. Each time a patient visits the doctor, an insurance claim is sent to the carrier for a payment. The claim must contains information about the visit such as the date, purpose and treatment cost. It would be possible for a patient to submit two claims on the same day.
Draw a class diagram based on system requirement described in the case study above.

3. A use case scenario description for a health club membership system is given below.

Every time a member enters the club, an attendant takes his or her card and scans it to make sure the person is an active member. If the member is not active, the system presents the amount of money to renew the membership.

The customer is given a chance to pay the fee and use the club, and the system makes note of the reactivation of the account so that special attention can be given to this customer when the next round of renewal notices are dispensed.
