The purpose of this life review paper is to interview J.B. regarding the various stages of his life including his childhood, adolescence, adulthood, family and his home life. The interview was conducted at his residence in North Olmsted. This life review paper is about a 65 year old Caucasian male. J.B. is a Roman Catholic and is of Italian decent. Client is currently residing with his wife of 40 years in North Olmsted, Ohio in a single family home with his dog, a Black Lab named Pongo. His son resided at his current residence in North Olmsted with him for 8 years and his daughter resided in his current residence for 11 years. J.B.’s daughter had a baby at the age of 21 out of wedlock and his grandson resided with him for 7 years. J.B.’s immediate family includes his 39 year old son, 38 year old daughter and 44 year old son-in-law. Client also has 9 grandchildren. His son has 2 girls’ ages 19 and 22, and fraternal twin boys age 14. His daughter has 4 boys ages 17, 10, 8, 8 and 1 daughter age 9. Client’s son J.B. resides in Rocky River, Ohio with his 4 children and his daughter B.B. resides in Berea, Ohio with her 5 children. C.B., the client’s wife who is 66 years old, is in good general health. She does have high blood pressure and high cholesterol which are both controlled with medications. She also suffers from osteoarthritis which has resulted in a right total shoulder replacement and right total knee replacement. Due to thin ligaments she suffers from torn rotator cuffs which cannot be corrected. She is a candidate to have a total left knee replacement and will consider this in the near future. S.F. is the grandfather of my boyfriend and this was the perfect opportunity to learn more about his family roots. It made for an easier interview because I was able establish more intimate connections by interviewing the grandfather of someone so close to me. S.F. is an 80 year old Caucasian male living with his wife of 59 years in Mineral Wells, Texas. Born in Brazos, Texas, he is the middle of five children. During his adolescent years, he worked on his family farm with his brothers and sisters while pursing education. He graduated from Texas Christian University with a bachelor’s degree in education. He then moved forward to the Air Force in hopes of becoming a pilot, but discovered at this time that he was color blind and could not continue on. Afterwards, he continued his education at Texas Wesleyan University and obtained his master’s degree in education. He later became a basketball coach and teacher in Weatherford ISD and eventually became school superintendent.
