Makiyah was referred to FACT on 9/21/17 by PerformCare for displaying youth was referred to FACT by PerformCare due to youth having suicidal thoughts. Makiyah wrote to a friend that, “she was stressed and that dying is a way out.” It was noted in the referral that Makiyah was experiencing significant symptoms of depression, difficulty sleeping, peer conflicts, isolating, presenting with depressed mood, and irritability. Makiyah reported feeling stressed due to not being able to go out with her friends when she wants to, have to focus on school, Makiyah also reports siblings' behavior as another stress due to their defiance. The family at the time of the referral was very guarded. The family at that time was open to in-home therapeutic (i.e. Individual and Family therapy) services. CM made a referral to CCS for IIC services. The family was assigned to Ms. Reid, IIC who worked well with the family. Ms. Reid was able to get Makiyah and Mrs. Mitchell to open up and really express their feelings and concerns. At one point in their session they discuss youth relationship with her father. Ms. Reid assist Makiyah with express her feeling and concerns about her relationship with her father. Ms. Reid attempted to bring Makiyah into their sessions but due to Makiyah father schedule and Makiyah having mixed feelings about her dad being a part of her sessions, youth father was not able to be a part of youth therapy sessions.
In November 2017 Makiyah was transported to Trinitas Hospital for screening due to suicidal ideation and leaving the house without permission. Youth was screened and sent home with a referral to Trinitas Hospital OP (i.e. Afterschool Program). Mrs. Mitchell attempted to call Trinitas Child & Adolescent Department to make an appointment for the Trinitas afterschool program but she was not able to get through.
