On 06/16/18 at 1900, I was receiving report on A. Mondragon from Leigh. She stated that the TPN was running out, so she dropped the rate from 23 mL/hr to 3 mL/hr and added Normal Saline to the TPN bag. This caught me off guard, especially since the new TPN and Lipids were sitting at the bedside. She stated the patient should still be receiving the TPN and that she just put the Normal Saline behind it, so it would run the remainder of the TPN to the patient. As soon as report was complete, I quickly ran the new TPN/Lipids and connected them to the patient at the proper rate. In my confusion during report, I did not ask how long the rate had been decreased, and no change was documented on the flowsheet. Therefore, I assumed, the rate had not
