The Performance Improvement Cycle

The Performance Improvement Cycle is an important element to the overall improvement of quality healthcare. In my opinion the process within the cycle that I consider the most important is the process of analyzing and comparing internal/external data. An example of this was given in the book where the hospital continuously monitored the delinquent health record rate. After doing this research they reached a benchmark which can also be called a standard of performance that the hospitals works to maintain. The successes and failures that come from working toward these standards allows the healthcare facility or hospital to improve on all aspects of healthcare performance. During the process of the performance improvement cycle the part where the most errors occured, in my opinion, has to be between the design and redesign process/education and the implement process/education time of the cycle. Like in any part of business, education or a creative venture I think the design and redesign process is a long period of indecision, agreements and disagreements. The implementation process/education can come with a lot of mistakes because you don't know what is going to work until you start to put it into effect. Educating your staff can pose a lot of challenges as well because every thing is brand new and needs to mastered. I think these errors can be avoided by implementing the changes slowly as well as having staff training sessions on a consistent basis. Performing test runs can also limit errors in the performance improvement process. An example can be by starting with one section of the facility or hospital seeing how it works and then branching out to different

...Performance Improvement: Joint Care Center Matthew Arless State University of New York Plattsburgh Blood loss in a total joint replacement surgery may subject the patient to allogeneic blood transfusion. This places a patient at risk for the potential of transfusion reactions, graft-versus-host disease, hyperkalemia, fluid overload, and infections.( Qintong 2014) Not to mention the fact that most patient’s satisfaction or even more significant avoidance of the surgery increases with the fear of receiving an allergenic blood transfusion. This in all sense and purposes is an organ transplant. Cost of an allogeneic blood transfusion and having a sufficient blood supply are also a factor. Reduction of blood transfusions post operatively is an area that presents an opportunity for improvement here at the Joint Care Center Champlain Valley Physician Hospital. The procedure that was traditionally implemented by most of the orthopedic surgeons at Champlain Valley Physician Hospital to reduce blood loss was with the use of a tourniquet during surgery to slow the loss of blood and the implementation of an auto transfusion device. This auto transfusion device would collect blood draining from the surgical site into a device that the nurses would then have to attach a PALL filter to for re-infusion back into the patient. This procedure would be treated under the transfusion protocol, using up valuable nursing time and with the added expense of the devices and restriction of the patients...

Chart Review

...Purpose. There are many reasons to conduct chart reviews such a required by a regulatory agency, justification of billing, and research. Another frequently used purpose of a chart review is to assist in measuring quality of care. This measurement is then used to help determine if a change in process is needed to improvement patient outcomes. There are several models used in process improvement including Juran’s Universal Sequence for Quality Improvement, the Seven-step problem-solving model, FOCUS-PDCA and Six-Sigma DMAIC (Langley, Moen, Nolan, Nolan, Norman & Provost, 2009). Chart reviews can be an integral step in all of these models in gleaning data to measure the depth of the problem or if it even exists. This paper will compare the advantages and disadvantages of reviewing charts, describe the basic considerations in performing chart reviews, and describe issues that can threaten the validity of data. Lastly, this paper will look closer at the Six-Sigma DMAIC process as this is the methodology used to guide process improvement in the facility for which this information is being obtained. ...training and development programs to assist employees in reaching their ultimate level of performance; however monitoring of performance is necessary to ensure individual and organizational goals are met. The process of assessing an employee’s performance, managing employee opportunities and identifying areas of weakness is evaluated and handled in the performance improvement plan (PIP). The PIP spells out the areas of need, determines goals to improve these areas, and develops training and educational needs associated with successful performance enhancement development. The PIP addresses both the areas of behavioral and performance. In the reflection, Team “D” will address training and development and the PIP process in more detail to gain a better understand of the process. ...Writing Assignment As commissioner of an agency responsible for child welfare, I am dedicated to ensuring that our caseworkers provide the children our agency protects with the highest level of support and aid. I believe that issues which affect the execution of our mission must be addressed promptly and attentively. I will use my influence in the agency to attend to concerns in a thoughtful manner. Internally, this includes managing cases well, improving caseworker training and skills, and increasing employee motivation through workshops and mentoring. Externally, I hope to keep our public image in check by properly attending to the case children and communicating with the media. Firstly, I will address the issue related to the two caseworkers reported to have not visited the families of a murdered eight year old and a twelve year old arrested for drug sales, in over three weeks. I will arrange for separate in person meetings at my office to discuss these situations in depth. This will provide the employees with an opportunity to express their perspectives and the potential causes for these tragedies. I plan to start the conversation by recognizing the good work they have done for the Agency in the past. Following this, I will transition into the fact that they have been noticeably absent as of late. The absenteeism will be addressed delicately--without prying into private life, instead by asking sincerely if there are issues at home in addition to asking...

How to Achieve Permanent, Measurable Improvements in Performance

...Permanent, Measurable Improvements In Performance Edit Article | Posted: May 11, 2007 The decades-long quest to transfer what is learned in the classroom to improved performance in the workplace has been a confounding one. For one thing, it isn't easy to change behavior patterns, because they've been ingrained over time. To correct performance issues, new behaviors have to be introduced and consistently reinforced over the long term. This means following up programs with enough continued learning, feedback, coaching and accountability to stimulate the growth of new neuronal connections that eventually take the place of old ones. Unaware of this reality, organizations have wasted billions of dollars annually investing in learning programs that fail to change behavior. The greater challenge of the quest for this "Holy Grail" is that the solution doesn't involve fixing just one thing. A myriad of variables within an organization influence whether learning is reinforced until new behavior patterns are established. A process I call Train-to-Ingrain was designed to achieve permanent, measurable improvements in performance by optimizing aspects of training and development that impact on learning transfer: - COMMITMENT - Support follow-up reinforcement - COACHING - Prepare direct managers for their development role - FOLLOW-UP - Integrate reinforcement programs with assessment and training programs - ACCOUNTABILITY - Measure performance improvement and calculate ROI ...
