Risk Assessment in medicine is conscientious evaluation of all the relevant factors, which help the clinician to predict an adverse event or outcome. It involves identifying the patient’s needs and condition on presentation to the clinician so as to draw up a care plan that put them at the most minimal risk. Even though it is immpossible to rule out all risk it is the clinicians duty to protect the patient to the maximum of their capacity and not put the patient in any unneccesary danger. (NHS 2007)

Description
It was towards the end of the day when a concerned mother turned up with her twelve month old infant who was asleep at the time and did not look too distressed. The baby’s mother explained how he had for the past couple of days gone off his milk and had had numerous episodes of vomiting. He was however still taking other food such as yoghurt and biscuits but also in much reduced quantities than before. The GP then did a full physical exam on the child and found he had no fever, rashes, altered breathing or significant dehydration. She then went on to reassure the mother that she was not overly concerned as she could not find anything worrying on her examination. She explained to the mother that as long as he had some food intake his appetite would eventually return to normal and she should keep encouraging him to take the food he would eat. At this point it was clear the mother was not happy and had anticipated so much more to be done. On realising this, the GP then offered the mom a return visit in the evening to check up on how the child had progressed during the day. She also advised her to go to the local A and E if she noticed anything distressing in the child’s overall condition. At this
